Technology of combined ointments

In pharmacy conditions, combined ointments are prepared in one mortar, if necessary, moving the previously obtained part of the ointment to the edge of the mortar.

Combined ointments are prepared in the following order: first, an ointment-suspension should be prepared, then an ointment-solution, and then an ointment-emulsion. At the same time, taking into account the presence of the resulting combinations, a different sequence of technological operations is possible, which should be rational.

The preparation of combined ointments is regulated by the same rules by which individual types of ointments are prepared. If the composition of the combined ointment includes medicinal substances that are insoluble neither in the base nor in water, then first of all, an ointment-suspension is prepared in a mortar ().

Liquids ( Vaseline oil or melted petroleum jelly) is used to dissolve hydrophobic substances, that is, an ointment solution () is obtained.

If water-soluble substances are prescribed in the preparation, then an ointment-emulsion is prepared ().

The widespread use of powders can be explained by their advantages over other dosage forms:

high pharmacological activity associated with fine grinding of medicinal substances;

ease of manufacture compared to tablets, pills;

portability and greater storage stability compared to liquid dosage forms;

the versatility of the composition, may include inorganic and organic substances, including powders of plant and animal origin, as well as small amounts of liquid and viscous substances;

dosing accuracy;

ease of use.

Powders also have a number of disadvantages:

slower action of medicinal substances compared to solutions, since the stage of absorption must be preceded by the stage of dissolution;

the possibility of changing the properties of certain substances under the influence environment as a result: loss of water of crystallization (sodium sulfate, sodium tetraborate, etc.); absorption of carbon dioxide from the air and transformation into other compounds (magnesium oxide turns into magnesium carbonate, etc.); interactions with atmospheric oxygen (oxidation of ascorbic acid) and moisture (dampening of analgin, dry belladonna extract);

substances with a bitter taste, odorous and coloring are inconvenient and unpleasant to take.

1.1 Powder preparation technology

All powders are classified as follows.

By application:

1. Pulveres ad usum internum - powders for internal use. These include most powders with a dosage of 0.1 to 1.0 per dose. 2. Pulveres ad usum externum -- powders for external use. These include:

tooth powders used for brushing teeth, whitening, pain relief and other purposes;

powders for the manufacture of solutions at home or in a hospital (rinses, lotions, etc.).

By composition. There are simple powders (one-component Pulveres simplices), consisting of one substance, and complex (Pulveres compositus), including several ingredients.

By the nature of dosing:

divided into separate doses (Pulveres divisi), dosing is done in a pharmacy;

undivided (Pulveres indivisi), are released to the patient in the total mass, and he independently performs dosing.

Divided powders can be prescribed in a distributive way: medicinal substances are prescribed for 1 dose, the number of doses is indicated; when calculating the total mass of medicinal substances, their quantity prescribed per dose is multiplied by the number of doses.

Dosed powders are more often intended for internal use, undosed - mainly for external use.

According to the nature of the impact on the body, powders are divided into powders of general and local action.

According to these factors, different requirements are placed on powders in terms of the degree of grinding, the method of preparation, packaging and dispensing.

In addition to their composition and method of administration, powders also differ from each other. state of aggregation(crystalline and amorphous), density, hardness, color, odor and other properties.

Animal powders and plant origin, as a rule, are amorphous and differ from powders of mineral origin in a lower density.

Some substances prescribed in powders change under the influence of light, oxygen, carbon dioxide and air moisture (chlorpromazine, apomorphine hydrochloride, yellow mercury oxide, cortisone acetate, etc.), hygroscopic (adrenaline hydrochloride, sodium chloride, urea, hexamethylenetetramine, etc.) , quite easily lose water of crystallization (sodium sulfate, magnesium sulfate), volatile (camphor, menthol, chloral hydrate, phenyl salicylate, etc.).

Sometimes, as a result of mixing several powdered substances, a chemical interaction between them can take place, a change in a number of the above physical properties composition, in some cases the formation of explosive mixtures, etc.

Grinding and mixing of medicinal substances in a pharmacy is carried out in mortars or various devices that allow mechanizing the process of preparing powders. Very often, both of these technological operations are performed simultaneously.

Grinding solid medicinal substances intended for powder preparation has a dual purpose:

this is necessary for a faster and more complete achievement of the therapeutic effect;

finely ground substances mix better, stratify less when dosing, and when examining a mixture of such substances with the naked eye, individual particles of ingredients are not detected, the presence of which is completely unacceptable.

The fulfillment of all these requirements is achieved by the fact that a decrease in the particle size of a substance during grinding leads simultaneously to an increase in the surface area of ​​the particles of this substance, which is called the total surface for short.

When calculating the total mass of powders, which include light (easy-moving, "dusty", with a low bulk density) medicinal substances, the number of the latter is doubled.

In the absence of special instructions in private pharmacopoeial articles, medicinal substances, in accordance with the requirements of SP XI, are crushed to a particle size of not more than 0.160 mm.

When mixing, fairly homogeneous mixtures are obtained when the amount of one ingredient does not exceed the amount of the other by 20 times (ratio 1:20). Therefore, when mixing at the same time, this ratio must be taken into account. If it is exceeded, the ingredient prescribed in more, in the process of preparation, are placed in the mortar first and in parts so that the ratio of 1:20 is not exceeded.

Dosing. Dividing the powder mass into individual doses is one of the critical operations in the technological process. Dosing of a mixture of medicinal substances is carried out by its mass using manual pharmacy scales. Before dosing, the entire powder is collected at the bottom of the mortar and visually checked for homogeneity. The weighing is carried out by adding the powder to the weighing pan using a capsule, a celluloid scraper or a scoop rolled out of paper.

Packing and decoration for the holiday. Powders not divided into doses are released in jars, bags and boxes; if they contain volatile, odorous, hygroscopic and weathering substances in jars sealed with corks.

The divided powders are usually dispensed in white writing paper capsules; powders with hygroscopic and weatherable substances - in waxed or waxed capsules, with volatile and odorous substances - in parchment capsules.

Hygroscopic substances are packed in waxed (charta cerata) or waxed (charta paraffinata) paper capsules, oily, volatile, odorous and adhesion-prone powders are packed in parchment paper capsules (charta pergamenta). Powders packaged in capsules are released to patients in cardboard boxes or flasks. Powders containing toxic substances are specially designed.

Preparation of the finished dosage form. In the manufacture of dosage forms according to individual prescriptions, written control passports are filled out. The passport indicates: date, prescription number, taken medicines and their quantity, number of doses; the signature of the person who manufactured, packaged and tested the medicinal product is put.

All calculations are made before the manufacture of the dosage form and are recorded on the back of the passport. The entry in the passport is made in Latin from memory immediately after the manufacture of the dosage form in accordance with the manufacturing technology. Passports of written control are stored in the pharmacy for two months.

The quality of powders is assessed according to the following indicators:

a) analysis of documentation;

b) correct packaging;

c) the correct design of the dosage form;

d) organoleptic control;

e) uniformity of powders;

e) flowability;

g) deviations in the mass of individual powders.

Powders may consist of medicinal substances with different physicochemical properties. The quantitative ratios of ingredients in complex powders often vary widely. It should be recalled that grinding in a mortar begins with a therapeutically indifferent ingredient (for grouting the pores of the mortar) or with the component, the loss of which during grinding is less. First, coarse-grained substances are triturated, and then finely crystalline ones.

If the ingredients in the composition of the powders are prescribed in different quantities, then the mixing begins with the preparations included in smaller quantities, gradually adding the remaining components. 1.2 Basic ways to overcome incompatibility

The problem of preventing pharmaceutical incompatibility of powders is part of the general problem of drug stabilization. The main ways to prevent incompatibility:

b) the introduction of excipients into the prescription of the medicinal product or a change in the composition of the prescription;

c) replacement of certain medicinal substances;

d) replacement of the dosage form.

a) This method is reduced to a certain sequence of dissolution (mixing) of the ingredients of a complex preparation. For example, a certain sequence of dissolution of components is recommended in the manufacture of mixtures, which include salts of alkaloids or synthetic, nitrogenous bases (salts of weak bases and strong acids) in combination with substances with alkaline reaction environment. In some cases, it is possible to avoid precipitation of alkaloid bases if the other components are added in the form of solutions in ascending order of their pH value.

Separate dissolution of medicinal substances in the part of the solvent, their separate mixing with part of the base or other components of the drug and the subsequent combination of the parts are used to prevent incompatibility in powders, liquid preparations for internal and external use, ointments, suppositories, injection solutions, eye drops, etc. LF.

b) Most cases of incompatibility are prevented by the use of various excipients as solvents, emulsion stabilizers, antioxidants, pH adjusters, moisture absorbers, ointment bases, etc.

c) Overcoming incompatibility by replacing: KBr >NaBr; codeine > codeine phosphate; caffeine > sodium benzoate > caffeine; sodium tetraborate > boric acid; liquid phenol > crystalline phenol; eufillin > theophylline.

d) Overcoming incompatibility by replacing potions with powders; drops on medicines; powders for medicines.

1.3 Physical and chemical incompatibility and ways to overcome it

Chemical incompatibilities are such incompatibilities that are accompanied by unforeseen chemical reactions of simultaneously prescribed drugs. This type of incompatibility is based on chemical reactions of varying intensity, resulting in the formation of inactive or inactive substances, and often poisonous ones. The nature of the interaction between medicinal substances can be very different and depends on the physicochemical properties of the substances, the type of dosage form, the pH of the dispersion medium and its ability to react with medicinal substances or promote interaction reactions.

Chemical incompatibilities can be caused by a wide variety of reactions, the rate of which depends especially on the type of dosage form and temperature regime. The greatest difficulty is the preparation of multicomponent injection solutions and eye drops that require heat sterilization, which speeds up slow reactions hundreds of times and often makes combinations compatible at room temperature incompatible.

Dampness and loss of flowability of complex powders are due to two main reasons:

1) the hygroscopicity of a mixture of substances is usually greater than the hygroscopicity of its constituent ingredients;

2) release of water as a result of a chemical reaction.

The first type of incompatibility is observed when the water vapor pressure of a mixture of powdered substances becomes less than the water vapor pressure of the environment. As a result, the mixture begins to attract moisture from the air. moreover, the mixture becomes more hygroscopic than each of its components separately. The dampening of complex powders is influenced by the following factors: the moisture content of the initial ingredients, the nature of their mixing, the relative humidity of the air in the room and the packaging material. The most significant influence is exerted by the relative humidity of the air in the room. Most damp mixtures lose their flowability and are moistened at a relative humidity of 50-60% and higher.

1) Diphenhydramine

At the same time, the mixtures blur and often turn yellow. Eufillin is incompatible with ascorbic acid, salts of weak bases and strong acids - diphenhydramine, dibazol, spasmolytin, etc.

To prevent dampening of powders with eufillin, it is proposed to add 3-5% of aerosil to them. The protective effect of aerosil is due to a mechanical obstacle to the interaction of reacting particles and the absorption of moisture by aerosil, which is formed during the interaction of the mixture components.

1.4 Packaging and presentation

For the packaging of dosed powders, in most cases, paper capsules are used from white smooth paper, factory-made or cut as needed (ex tempore) in the form of rectangles measuring 7.5X10 cm.

Hygroscopic substances are packed in waxed (charta cerata) or waxed (charta paraffinata) paper capsules, oily, volatile, odorous and adhesion-prone powders are packed in parchment paper capsules (charta pergamenta).
