Pharmacodynamics
The molecular weight of the complex is so high - about 50 kDa - that its diffusion through the mucosa of the gastrointestinal tract is 40 times slower than the diffusion of ferrous iron. The complex is stable and does not release iron ions under physiological conditions. The iron of the multinuclear active zones of the complex is bound into a structure similar to the structure of the natural iron compound - ferritin. Due to this similarity, the iron (III) of this complex is absorbed only by active absorption. Iron-binding proteins found on the surface of the intestinal epithelium and in the gastrointestinal fluid absorb iron(III) from the complex through competitive ligand exchange. Absorbed iron is mainly deposited in the liver, where it binds to ferritin. Later in the bone marrow, it is incorporated into hemoglobin.
The polymaltose complex of iron (III) hydroxide, unlike iron (II) salts, does not have prooxidant properties. The sensitivity of lipoproteins (eg, very low density lipoproteins and low density lipoproteins) to oxidation is reduced.
Pharmacokinetics
Studies using the double isotope method (55 Fe and 59 Fe) showed that iron absorption, measured by the level of hemoglobin in red blood cells, is inversely proportional to the dose taken (the higher the dose, the lower the absorption). There is a statistically negative correlation between the degree of iron deficiency and the amount of iron absorbed (the higher the iron deficiency, the better the absorption).
The maximum absorption of iron occurs in the duodenum and jejunum. Unabsorbed iron is excreted in the faeces. Its excretion with exfoliating cells of the epithelium of the gastrointestinal tract and skin, as well as with sweat, bile and urine, is approximately 1 mg of iron per day.
In women during menstruation, there is an additional loss of iron, which must be taken into account.

Contraindications for use

Hypersensitivity to the components of the drug,
iron overload of the body (for example, in cases of hemochromatosis, hemosiderosis);

In the course of controlled studies in pregnant women (2nd and 3rd trimesters of pregnancy), there were no negative effects on the mother and fetus. There was no harmful effect on the fetus when taking drugs in the first trimester of pregnancy.
Breast milk of a nursing mother normally contains iron associated with lactoferrin. There are no data on the amount of iron that enters mother's milk from the complex with polymaltose. However, the occurrence of undesirable effects in breastfed children is unlikely.

Dosage and administration

Inside, tablets can be chewed or swallowed whole. The drug is recommended to be taken during or immediately after a meal.

Interaction with other drugs

Interaction with other drugs or food has not been identified.
Simultaneous use with parenteral iron preparations and other oral iron (III) preparations of polymaltose hydroxide is not recommended due to a pronounced inhibition of the absorption of orally ingested iron.

DESCRIPTION

The packaging of the iron-containing preparation Ferrum Lek is a small square box of a typical color for this company.

Inside the box are the ampoules themselves in the amount five pieces (the contents of two ampoules at the time of photographing were already wandering inside my body), which are embedded in a substrate with individual cells. In this package, the ampoules are completely safe.

I am not a doctor, so I will not succeed in explaining the mechanism of action of this complex in the right words. The only thing I understood is that thanks to this form, there is a rapid replenishment of iron stores in the body.

My testimony

Iron deficiency anemia has haunted me for many years. With varying success, I either normalize my hemoglobin level, or it rolls down again. Diagnosis is made and then removed. In one of my reviews, I already wrote about the Fenuls iron-containing drug (you can read it), with the help of which I try to maintain hemoglobin.

In the last six months, I somehow started the situation by stopping taking the above drug, and I was in no hurry to do a blood test either. But the circumstances developed in such a way that at the end of September I still had to run around to the doctors, and at the same time pass a bunch of tests.

As expected, hemoglobin levels were again below normal. The therapist, after seeing the results, shook his head and threatened with injections. Thought he was joking, but no. Despite the fact that the hemoglobin level was low, but far from critical, he prescribed me a five-day course of injections of Ferrum lek 50 mg, to quickly raise hemoglobin, and only then drink Fenyuls for two months, to accumulate iron reserves in the body.

Despite the fact that the instructions describe what dosage of Ferrum Lek is necessary for each specific case, only a doctor prescribes this drug, yes besides it is released by prescription. At the pharmacy they asked me for it, and they didn’t even want to sell it due to the lack of some kind of seal there. They also asked for the recipe at the clinic where I went to do them.

APPLICATION FEATURES

The therapist immediately warned that injections should be done in a hospital if there is no health worker at home. Even the manual says:


This is due to the fact that the injection is given intramuscularly, i.e. in the buttock, and from improper administration, bruises that do not go away for a long time can form. In addition to the aesthetic side of the issue, iron preparations in such a concentration can cause unforeseen allergic reactions.

After listening to all this, I decided not to risk it and get injections at the nearest clinic. Of course, the prices are not encouraging: 200 rubles per injection. Thus, a thousand rubles for injections were added to the cost of the drug itself.

In addition to the drug itself, I needed to purchase syringes. Since the injection must be done deep into the muscle, the pharmacy gave me 5 ml syringes, they have just the right length of the needle. If you still decide to inject yourself, then consider this point.

Feelings during and after injection

I am not afraid of injections. Well, I’m not afraid, I don’t faint from the sight of a needle. This is a plus, because. I don’t shrink from fear, and the needle enters the target like clockwork

If you don't stress, then injection is completely painless . A couple of times I didn’t even realize that everything was already done. By the way, the Ferrum Lek injection itself is not done instantly, but the contents are injected slowly, about 1-2 minutes, although according to the instructions:

You get tired of standing for so long.

The injection site begins to ache half a minute after the injection of the drug, but not much, very tolerable. After 10-20 minutes, all discomfort disappears.

Side effects from the use of Ferrum Lek

Fortunately, I did not have any side effects from Ferrum Lek injections.. The only thing that still took place was the minimum bruises in the amount of 2 pieces and the size of a penny. At the time of writing the review, and this is 10 days after the last injection, the bruises have practically disappeared, but the injection site still hurts as you press on it.

How to Minimize Bruising and Speed ​​Up Healing

First of all, the likelihood of bruising depends on the correct administration of the drug and on the skill of the medical staff (if you do it in a hospital). If the injection is delivered as it should, then you can completely do without this side effect. But it is better to take the initiative in your own hands and take measures to prevent them.

Firstly, after the injection you need to move. Due to the fact that I did the injections outside the house, it worked out for me, 20 minutes of intense walking to the house.

Secondly, I used Troxevasin gel at the injection site. I smeared several times a day throughout the course, and after its completion, until the bruises almost completely disappear. You can use any absorbable ointment / gel.

RESULTS

A week after the completion of the five-day course, I went to take a blood test. The result pleased, the level of hemoglobin and other indicators improved. Hemoglobin slightly fell short of the norm, but it's not scary.


I note that a month passed between the two tests, during which a surgical intervention was performed in my body, which was accompanied by a certain blood loss. Consequently, the level of hemoglobin at the time of the appointment of Ferrum Lek injections was even lower than indicated in the analysis in the top photo.

Thus, Ferrum Lek managed to improve the performance in a short period of time, namely, in just 5 days. But it’s too early to relax, there are still 2 months of taking Fenyuls ahead.

CONCLUSIONS

Despite the fact that I was completely satisfied with the action of Ferrum Lek, one should not hope that he will solve the problem of iron deficiency anemia once and for all, because. since it has already appeared, then, most likely, it will periodically pop up as soon as you relax both in terms of nutrition and in terms of prevention.

Ferrum Lek injections are effective for quickly raising the level of hemoglobin, but for the accumulation of iron stores, it is necessary to take other forms of iron-containing preparations.

Once again, I would like to emphasize that you cannot prescribe injections to yourself. Only a doctor can determine whether you need them or not, and also accurately prescribe the dosage, because. An excess of iron is no less dangerous than its deficiency.

On my own behalf, I can only say that if you have iron deficiency anemia, or you are in the category of an increased risk of its development, then in addition to complete blood count, hand over a separate analysis on ferritin. Doctors in most cases do not prescribe this analysis, because. they don't do it for free. Even with good hemoglobin level, ferritin levels may be low, and this already indicates the development of iron deficiency anemia, and prophylaxis should already be prescribed.
