Pharmacological properties:

Pharmacodynamics. A drug that compensates for the deficiency of nicotinic acid (vitamin PP, B3); exhibits vasodilating, hypolipidemic and hypocholesterolemic action.
Nicotinic acid and its amide (nicotinamide) is a component of nicotinamide adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate (NADP), which play an essential role in the normal functioning of the body. NAD and NADP - compounds that carry out redox processes, tissue respiration, carbohydrate metabolism, regulate the synthesis of proteins and lipids, the breakdown of glycogen; NADP is also involved in phosphate transport.
The drug is a specific antipellargic agent (nicotinic acid deficiency in humans leads to development).
It has a vasodilating effect (short), including on the vessels of the brain, improves microcirculation, increases the fibrinolytic activity of the blood and reduces platelet aggregation (reduces the formation of thromboxane A2).
Inhibits lipolysis in adipose tissue, reduces the rate of synthesis of very low density lipoproteins. Normalizes the lipid composition of the blood: reduces the level of triglycerides, total cholesterol, low density lipoproteins, increases the content of high density lipoproteins; has an anti-atherogenic effect.
Has detoxifying properties. It is effective in Hartnup's disease - a hereditary disorder of tryptophan metabolism, accompanied by a deficiency in the synthesis of nicotinic acid.
Nicotinic acid has a positive effect on the duodenum and enterocolitis, sluggishly healing wounds and ulcers, diseases of the liver, heart; has a moderate hypoglycemic effect.
Promotes the transition of the trans-form of retinol to the cis-form used in the synthesis of rhodopsin. It promotes the release of histamine from the depot and the activation of the kinin system.

Pharmacokinetics. It is metabolized in the liver by amidation, followed by the formation of nicotinamide mononucleotide, the precursor of NAD. Further phosphorylation of NAD produces NADP. NAD and NADP are unevenly distributed in the body: most of all in the liver (depositing organ), then in the brain, heart muscle, kidneys, skeletal muscles and in the blood (erythrocytes). Care must be taken when combined with antihypertensive drugs (possibly increased hypotensive action), anticoagulants, acetylsalicylic acid (due to the risk of hemorrhages).
Reduces the toxicity of neomycin and prevents the decrease in the concentration of cholesterol and high density lipoproteins induced by it. Weakens the toxic effect of barbiturates, anti-tuberculosis drugs, sulfonamides.
Oral contraceptives and isoniazid slow down the conversion of tryptophan to nicotinic acid and thus may increase the need for nicotinic acid.
Antibiotics can increase the flushing caused by nicotinic acid. Symptoms: increased side effects from the cardiovascular system - dizziness, feeling of a rush of blood to the head. Treatment: drug withdrawal, detoxification therapy, symptomatic treatment. There is no specific antidote.

Pharmacokinetics

Nicotinic acid is rapidly absorbed when administered parenterally. It is evenly distributed over organs and tissues. It is inactivated mainly by methylation and less by conjugation. Partially biotransformed in the liver with the formation of N-methylnicotinamide, methylpyridonecarboxamides, glucuronide and a complex with glycine. The half-life (T 1/2) is 45 minutes. Excreted from the body by the kidneys in unchanged form and in the form of metabolites. Renal clearance depends on the concentration of nicotinic acid in plasma and may decrease at high plasma concentrations.

Pharmacodynamics
The structure of nicotinic acid is close to nicotinamide.
Nicotinic acid and its amide play an essential role in the life of the body: they are prosthetic groups of enzymes - codehydrase I (diphosphopyridine nucleotide - NAD) and codehydrase II (triphosphopyridine nucleotide - NADP), which are hydrogen carriers and carry out redox processes. Codehydrase II is also involved in the transfer of phosphate.

Dosage and administration
Nicotinic acid is administered to adults subcutaneously, intramuscularly or intravenously slowly.
For intravenous jet administration a single dose of the drug is diluted in 10 ml of 0.9% sodium chloride solution, injected at least 5 minutes before (no faster than 2 mg of nicotinic acid per 1 minute).
For intravenous drip a single dose of the drug is diluted in 100-200 ml of 0.9% sodium chloride solution, the rate of administration is 30-40 drops per minute.
Subcutaneous and intramuscular injections are painful.
With pellagra, a 1% solution of 1 ml is administered intravenously or intramuscularly 1-2 times a day for 10-15 days.
In case of ischemic cerebrovascular accident, 1 ml of a 1% solution is administered intravenously (slowly).

Drug Interactions
Oral contraceptives and isoniazid reduce the conversion of tryptophan to nicotinic acid and thus may increase the need for nicotinic acid.
Nicotinic acid reduces efficacy and toxicity barbiturates, anti-tuberculosis drugs, sulfonamides. When used together with sulfonylurea drugs, it can increase blood glucose levels. Also, nicotinic acid reduces the toxicity of neomycin and prevents the decrease in the concentration of cholesterol and high density lipoproteins induced by it.
Antibiotics may increase the redness of the skin caused by nicotinic acid.
Acetylsalicylic acid reduces the effect of reddening of the skin that occurs under the action of nicotinic acid.
Ciprofibrate it is not recommended to combine with nicotinic acid.
lovastatin, pravastatin due to the increased risk of adverse reactions, it is not recommended to combine with nicotinic acid.
Care must be taken when combined with antihypertensive drugs(may increase hypotensive effect), anticoagulants, acetylsalicylic acid(due to the risk of developing hemorrhages).

Intravenously administered slowly. Subcutaneous and intramuscular injections of nicotinic acid are painful. Sodium nicotinate (nicotinic acid sodium salt) or nicotinamide can be used to avoid irritation.
Higher doses for adults inside: single - 0.1 g, daily - 0.5 g; in a vein (in the form of sodium salt): single - 0.1 g, daily -0.3 g. Nicotinic acid contraindications:
Intravenous injections are contraindicated in severe forms of hypertension (persistent increase in blood pressure) and atherosclerosis.
Persons with increased susceptibility to nicotinic acid should be prescribed nicotinamide, except in cases where nicotinic acid is used as a vasodilator.
