Almagel is a drug that reduces the increased acidity of gastric juice. It is a balanced combination of aluminum and magnesium hydroxides, which are insoluble and hardly enter the body. Almagel has a local effect on the gastric mucosa, protects it from the irritating effect of acids and other harmful substances and food, and reduces the activity of pepsin. The drug reduces the acidity in the esophagus. It has an anti-inflammatory and astringent effect on the mucous membrane of the esophagus, stomach and duodenum. The auxiliary substance sorbitol has a weak carminative and moderate choleretic effect, as well as a moderate laxative effect.

Indications for use

Symptomatic treatment of diseases of the digestive tract, accompanied by increased acidity of gastric juice: esophagitis, hiatal hernia, gastroesophageal reflux disease (GERD), acute and chronic gastritis, gastroluodenitis, peptic ulcer of the stomach and duodenum, post-resection gastritis and gastroanastomositis in the presence of gastroesophageal reflux (heartburn). ). Prophylactically to reduce the irritating and ulcerogenic effect of certain drugs on the mucous membrane of the esophagus, stomach and duodenum (for example, corticosteroids, non-steroidal anti-inflammatory drugs).

Contraindications

The use of Ashiagel is not recommended in case of hypersensitivity to the drug components, habitual constipation, Alzheimer's disease, severe abdominal pain of unspecified origin, suspected acute appendicitis, ulcerative colitis, colostomy or ileostomy, chronic diarrhea, hemorrhoids, severe renal failure , hypophosphatemia, during lactation, children under 10 years of age.

Application Precautions

Patients need to see a doctor in case of weight loss, difficulty in swallowing or constant discomfort in the abdomen, digestive disorders that appeared for the first time; or when changing the course of existing digestive disorders.

Aluminum hydroxide can lead to constipation and magnesium hydroxide can lead to intestinal hypokinesia. The use of this product in high doses may cause or worsen bowel obstruction and ileus, especially in patients at increased risk of such complications, such as patients with renal insufficiency or elderly patients.

The use of antacids containing aluminum in elderly patients should be limited.

With prolonged use of the drug, it is necessary to ensure that a sufficient amount of phosphorus enters the body, since aluminum hydroxide binds to phosphates and reduces their absorption from the digestive tract. The excretion of calcium in the urine increases, which can lead to disturbances in the calcium-phosphate balance and create conditions for the development of osteomalacia (symptoms are complaints of weakness and pain in the bones).

Aluminum hydroxide can be hazardous when used by porphyria patients on hemodialysis.

Since aluminum is concentrated in nervous tissue, the use of aluminum-containing antacids in elderly patients and patients with Alzheimer's disease should be strictly limited. When taken by elderly patients, the course of existing diseases of the bones and joints may worsen.

With prolonged use of the drug (more than 14 days), regular medical supervision and determination of the magnesium content in the blood is necessary in the treatment of patients with renal insufficiency.

Interaction with other medicinal products and other forms of interaction

Use during pregnancy and lactation

There are no clinical data on the use of Almagel by pregnant women. Information about excipients

The drug contains sorbitol, which allows it to be taken by patients with diabetes, but is not suitable for the treatment of patients with congenital fructose intolerance, as it can cause stomach irritation and diarrhea.

The suspension contains parahydroxybenzoates, which can cause an allergic reaction (possibly of a delayed type) and very rarely - bronchospasm.

The drug contains 2.5 vol.% ethyl alcohol (ethanol), which corresponds to the content of 98.1 mg of ethanol in 5 ml and 196.2 mg of ethanol in 10 ml of the drug. Due to the presence of ethyl alcohol in the composition of Almagel, this drug should be used with caution by pregnant, lactating, children and patients with liver disease, epilepsy and alcohol dependence.

Dosage and administration

Symptomatic antacid treatment

Adults and children over 14 years of age

Children from 10 to 14 years old

Apply at a dose equal to half the adult dose - 1 scoop 2-4 times a day or 2 scoops 1-2 times a day or 1 packet 1-2 times a day.

For prevention

5-15 ml (1-3 scoops or 1 packet) 15 minutes before taking irritant medications.

Before each dose, the suspension must be thoroughly homogenized by shaking the package!

The drug is taken 45-60 minutes after a meal and in the evening before bedtime.

Overdose

There may also be signs of metabolic alkalosis: changes in mood or mental activity, numbness or muscle pain, nervousness and fatigue, slow breathing, unpleasant taste sensations.

In these cases, it is necessary to immediately take measures to quickly remove the drug from the body - gastric lavage, stimulation of vomiting, activated charcoal.

Like all medicines, Almagel can cause side effects, although not everybody gets them.

From the gastrointestinal tract:

Laboratory indicators:

Almagel ® Neo is a balanced gel of aluminum hydroxide and magnesium hydroxide in combination with the vitamin component - simethicone. It has a long antacid effect, has a mild laxative effect, successfully eliminates constipation and reduces flatulence in patients with stomach ulcers.

The dual mechanism of its action, combining acid-neutralizing ability and cytoprotective activity, provides protection of the gastric mucosa and its healing in case of damage.

Alkaline active components of the drug are finely dispersed, which increases their active surface, provides closer contact with the surface of the mucous membrane of the stomach and duodenum, slows down the evacuation of the contents of the stomach and lengthens the neutralizing effect.

Cytoprotective activity is associated with the activation of the synthesis of prostaglandins, which stimulate the secretion of mucus and bicarbonates of the gastric mucosa.

Given the ability of the drug to bind bile acids and lysolecithin, as well as to have a gastroprotective effect, Almagel ® Neo can be used to treat duodenogastric reflux.

Simethicone, which is part of the drug, helps to eliminate flatulence.

Simethicone is a stable silicone substance that is not absorbed from the digestive canal, causing the natural release of gases and their absorption by the intestinal walls.

Simethicone exerts its action in the lumen of the small intestine and is excreted unchanged from the body.

The constituents of the drug are not absorbed in the gastrointestinal tract and thus do not lead to clinically significant plasma concentrations when used at the recommended doses and for the recommended duration of treatment.

Aluminum hydroxide neutralizes the increased secretion of hydrochloric acid in the stomach, forming aluminum chloride. Under the influence of the alkaline environment of the intestine, the latter is converted into alkaline aluminum salts, which are poorly absorbed and excreted in the feces. With normal kidney function, the serum level of aluminum remains practically unchanged.

Magnesium hydroxide also neutralizes hydrochloric acid in the stomach and turns into magnesium chloride. In the intestines, it turns into magnesium carbonate, which is poorly absorbed, so the concentration of magnesium ions in the blood practically does not change.

Indications for use

Symptomatic treatment of diseases of the digestive tract, which are accompanied by increased or normal acidity of gastric juice and increased gas formation.

The drug is used alone or in combination with other drugs for:

Contraindications

Dosage and administration

Adults and children over 14 years of age:

Overdose

Prolonged use of Almagel ® Neo can cause hypermagnesemia, despite the fact that the drug is almost not absorbed from the alimentary canal. Symptoms of an overdose are manifested by rapid fatigue, reddening of the face, exhaustion, muscle weakness and inappropriate behavior.

There may also be signs of metabolic alkalosis: changes in mood or mental activity, numbness or muscle pain, nervousness and fatigue, slow breathing, unpleasant taste sensations.

If an overdose of the drug is suspected or if clinical signs appear, it is necessary to stop the drug and take measures to quickly remove the drug from the gastrointestinal tract (introduce activated charcoal, do gastric lavage and other procedures to reduce the absorption of aluminum and magnesium ions).

Treatment for magnesium overdose: rehydration, forced diuresis. You can prescribe calcium gluconate intravenously. In case of renal insufficiency, hemodialysis or peritoneal dialysis is necessary.

Side effects

Prolonged use of Almagel ® Neo or taking high doses can cause phosphorus deficiency syndrome (loss of appetite, muscle weakness, weight loss).

It should be borne in mind that even when using the usual doses of the drug in patients whose diet is characterized by a low phosphorus content, it is possible to reduce the phosphorus content in the body, increase the processes of resorption in bone tissue, hypercalciuria, osteomalacia, osteoporosis, since this drug contains aluminum (see. section "Proper precautions for use"). In patients with renal insufficiency, elevated plasma concentrations of both aluminum and magnesium are observed, therefore, long-term use of high doses of aluminum and magnesium salts can lead to the development of encephalopathy, dementia, microcytic anemia, or worsen the course of dialysis-induced osteomalacia.

Aluminum hydroxide can be dangerous for patients with porphyria who are receiving hemodialysis.

Adverse reactions:

  • ;from the gastrointestinal tract: taste disturbance (taste of chalk), diarrhea, constipation, nausea, vomiting, discoloration of feces;

Interaction with other medicinal products and other forms of interaction

Almagel ® Neo interacts with some other drugs that are taken orally. There is a decrease in the volume of absorption of drugs that are taken simultaneously from the gastrointestinal tract. As a measure, a break should be taken between taking antacids and other drugs.

Recommended to be taken 2 hours before or 2 hours after use of the drug Almagel ® Neo: antibiotics of the tetracycline group (tetracycline, doxycycline), fluoroquinolones (ciprofloxacin, norfloxacin, ofloxacin), H 2 -antihistamines, anti-tuberculosis drugs: ethambutol, isoniazid (oral), atenolol, metoprolol, propranolol, chloroquine, cyclini, diflunisal, digoxin, diphosphonates, fexofenadine, iron (salts), sodium fluoride, glucocorticoid drugs (described interaction with prednisolone and dexamethasone), indomethacin, kayexalate, ketoconazole, lansoprazole, lincosamide, phenothiazine antipsychotics, penicillamine, phosphorus (additives), thyroxine.

Combinations to consider: with simultaneous use with salicylates, the excretion of salicylates by the kidneys increases due to alkalinization of urine.

The drug should not be used simultaneously with quinolines.

Simultaneous use with cholinergic agents reduces their effectiveness.

The simultaneous use of aluminum hydroxide and citrates can lead to an increase in the level of aluminum, especially in patients with renal insufficiency.

The use of the drug together with drugs that have an enteric membrane may lead to more rapid dissolution of the membrane and irritation of the stomach and duodenum.

The drug may reduce the absorption of folic acid.

When combined with levothyroxine, it is possible to reduce its hormonal action. Pirenzepine enhances and prolongs the action of Almagel ® Neo.
