Description

Vascular lesions of the optic nerve occur with changes in the vessels that feed the optic nerve, as well as with pathology of the cerebral vessels. They are described under various names: acute circulatory disorders in the system of arteries supplying the optic nerve, arteriosclerotic papillitis, ischemic edema, optical malacia, disc edema due to diseases of the optic nerve arteries, atherosclerotic neuritis, ischemic neuropathy, anterior ischemic neuropathy.

These terms appeared at the first stages of studying the vascular pathology of the optic nerve and reflect only one side of the disease process. So, some of them indicate damage only to the arteries of the optic nerve, while there are also venous disorders, others (ischemic neuropathy, anterior ischemic neuropathy) emphasize the presence of ischemia in the fundus.

But the latter is not always observed in vascular disorders in the optic nerve. In some cases, the optic disc may be hyperemic or remain within the normal range. All these designations cannot be considered sufficiently precise. S. F. Shereshevskaya et al. (1981) circulatory disorders in the optic nerve are called vascular optic neuropathy. This term, although generalizing, is more objective; it is more expedient to use this definition in the clinic.

Optical neuropathy.

Etiology .

The causes that cause damage to the vessels of the optic nerve are mainly general vascular diseases: atherosclerosis, hypertension and hypotension, temporal arteritis, periarteritis nodosa, arteritis obliterans, diabetes mellitus, disorders in the vertebrobasilar system with discopathy of the cervical spine, thrombosis of the main vessels.

Pathogenesis.

The basis of acute circulatory disorders in the optic nerve can be both functional vascular disorders (spasms) and organic changes. Organic changes have been morphologically studied mainly in atherosclerosis and temporal arteritis. M. I. Merkulova (1962), N. Piper and L. Unger (1957), W. Peters (1958), S. DukeElder (1971) showed that against the background of general atherosclerosis, sclerotic changes may also develop in the vessels of the optic nerve. Certain disorders of hemodynamics and coagulation properties of the blood can cause the formation of a blood clot.

Thrombus formation leads to vascular occlusion, to a deterioration in the blood supply to the optic nerve and to the subsequent death of nerve fibers due to the appearance of softening foci in them, nested gliosis and proliferation of connective tissue.

With temporal arteritis, an inflammatory process is observed in the wall of the temporal artery: a giant cell infiltration occurs, which is gradually replaced by connective tissue. S. Heyrch (1974), using the method of fluorescent angiography, came to the conclusion that occlusive processes in the posterior ciliary arteries are of primary importance in the pathogenesis of optic vascular neuropathy.

Partial occlusion of the posterior ciliary arteries, as well as severe general hypotension, large blood loss can lead to a decrease in perfusion pressure. A drop in perfusion pressure causes a violation of hemocirculation, primarily in the vessels of the optic disc, in non-ripanillary choroidal vessels, and then in the entire vascular system of the choroid.

With temporal arteritis, stenosis of the ophthalmic artery and impaired capillary circulation in the retrobulbar region occur.?

Clinic.

Diagnosis of vascular disorders in the optic nerve is based on anamnestic data, information about the state of the fundus and the results of functional studies. Among the latter, a particularly important place is occupied by the definition of the field of view. There are two forms of vascular lesions of the optic nerve - arterial and venous, each of which can occur acutely and chronically.

Acute violation of arterial circulation is characterized by a sudden decrease in visual acuity or the appearance of defects in the visual field, which often occur after sleep, physical or emotional stress, and sometimes with complete well-being.

Visual acuity with arterial discirculation can be sharply reduced to the movement of the hand near the face, hundredths, or remain within tenths. In the field of view, changes are determined in the form of prolapse of the lower sections, lower inner quadrants, various defects in the upper halves of the field of view, central and paracentral scotomas, concentric narrowing.

In patients with impaired arterial circulation in the optic nerve, treated at the clinic of eye diseases of the Ukrainian Institute for Postgraduate Medical Education, both hemianopic or quadrant restrictions in the lower or upper halves of the visual field and central or paracentral scotomas were equally often observed, concentric narrowing of the visual field was less common. On fig. 102, 103, 104 shows variants of visual field defects in patients under our supervision.

In the fundus, edema and severe ischemia of the optic nerve head, narrowing of arterial vessels, and sometimes concomitant hemorrhages are more often detected (Fig. 105). In some patients, the fundus may be normal or there may be mild disc edema without ischemia.

Yes, from 76 patients treated in our clinic for acute disorders of arterial circulation in the optic nerve, 19 the optic disc did not present any abnormalities or there was a slight edema without ischemia. Acute violation of arterial circulation in such cases is diagnosed on the basis of a sudden decrease in visual acuity, characteristic defects in the visual field and the result of additional examinations.

Some features of changes in the fundus were noted in temporal arteritis (Hortop's disease). In the early stage of this disease, congestive hyperemia of the optic disc appears, turning into a pronounced vitreous edema; the latter is often accompanied by the deposition of a milky white exudate on the disc.

Quite often, vascular edema of the optic nerve head in temporal arteritis is combined with obstruction of the central retinal artery or its branches. Usually the pathological process proceeds malignantly.?

Acute violation of arterial circulation in the optic nerve ends with rapidly developing partial or complete atrophy of the optic nerve. Within weeks or months to 1-2 years and later, usually similar changes in the optic nerve appear in the other eye.

Since optic nerve atrophy is determined at the bottom of the previously affected eye, and vascular edema resembling a congestive disc is determined at the bottom of the other eye, the disease can be mistakenly regarded as Kennedy's syndrome, but a detailed history and general examination data can avoid this mistake.

In acute disorders of venous circulation, visual acuity, as in arterial disorders, is reduced to varying degrees (from tenths to light perception). But the dynamics of vision loss with changes in the venous system differs from that with arterial disorders. According to our observations, with venous circulation disorders, a mild decrease in visual functions may initially be observed, but after 1-2 day there is a progressive deterioration of them to stable limits.

With arterial discirculation, as mentioned above, a decrease in visual acuity appears suddenly. This position can be used as one of the differential diagnostic signs of impaired arterial and venous circulation in the optic nerve. Typical defects in the visual field in venous circulation disorders are central or paracentral scotomas, concentric narrowing of the visual field, prolapse of the type of horizontal hemia

With ophthalmoscopy, the optic nerve disc appears slightly edematous, retaining a pink color or hyperemic (dashed retinal hemorrhages are located around it). The veins are often dilated, along their course there may also be hemorrhages (Fig. 106). The arteries are of normal caliber or narrowed. The process ends with atrophy of the optic nerve.

Chronic vascular insufficiency is characterized by a slow gradual development of the disease and is manifested by a decrease in visual acuity, often pronounced edema of the optic nerve head, and narrowness of the arteries. The disease ends with atrophy of the optic nerves.

As chronic vascular insufficiency S. Heureh (1976) considers atrophy of the optic nerve in low pressure glaucoma. It occurs as a result of discirculation in the posterior ciliary arteries due to an imbalance between perfusion and intraocular pressure. The drop in perfusion pressure in the posterior ciliary arteries may be associated with local or systemic vascular disorders, such as arterial hypotension, embolic phenomenon, hematogenous disorders.

Vasculitis of the optic nerve.

Among the vascular pathology, vasculitis, described by S. Heyreh (1972) and others, deserves attention.

Etiology

disease has not been fully elucidated. It is believed that it is based on an inflammatory-allergic reaction to bacteria, viruses and various antigenic substances. Confirmation of this assumption is the effectiveness of corticosteroids.

Clinic .

Optic vasculitis occurs at a young age and is unilateral. Visual acuity does not decrease sharply. Peripheral vision is not affected. Ophthalmoscopically, it can manifest itself in two forms. In the first form, the picture resembles a congestive disc, but the lack of data indicating CSF hypertension and unilateral disease does not support this diagnosis.

In the second form, multiple hemorrhages along the vessels are observed in the fundus, as in thrombosis of the central retinal vein, but not so extensive. There are "couplings" along the vessels and small "lumps" of white exudate. The development of the first form is explained by vasculitis of the disc vessels, which leads to increased capillary permeability and to its edema.

In the second form, phlebitis of the central retinal vein develops in the region of the head of the optic nerve or the retrobulbar part, which causes localized thrombosis of the central vein; in such cases, the phenomena of stasis prevail over the edema of the disc. The prognosis is favorable. V. I. Kobzeva and M. P. Pronin (1976) described 5 patients with optic vasculitis.

The lesions of the optic nerve caused by the pathology of the intracranial vessels include its changes in the amaurosis-hemiplegic Elynnig-Merkulov syndrome, which is quite common in clinical practice.

The syndrome consists of amaurosis or decreased vision in one eye and movement disorders (hemiplegia, hemiparesis) on the opposite side. It is observed in patients with atherosclerosis, hypertension, rheumatism. Amaurosis-hemiplegic syndrome develops when blood circulation is disturbed in the central retinal and middle cerebral arteries.

The cause of the development of the Elynnig-Merkulov syndrome is most often thrombosis of the internal carotid artery. At the same time, blood flow in the arterial circle of the brain increases, vortex juices are created near the thrombus in the internal carotid artery, which contribute to the separation of pieces from the thrombus. The latter can close the lumen of the central retinal or middle cerebral arteries.

The clinical picture of the cross amaurosis-hemiplegic syndrome may be due to the re-irritation of the carotid sinus located in the area of ​​the bifurcation of the common carotid artery. In this case, visual and motor disorders develop as a result of vasospasm or regional hypotension and are transient.

The disease is more often manifested by changes in the organ of vision. After a few days or months, movement disorders join them. There are motor, and then visual disturbances. Only sometimes visual and motor disorders develop simultaneously.

As a rule, patients note first of all unilateral decrease in vision in varying degrees. In the field of view, various changes are found depending on the location and severity of the process. Ophthalmoscopically, there is mainly a picture of vascular optic neuropathy or acute obstruction of the central retinal artery, leading to atrophy of the optic nerve.

There are cases of transient visual impairment from a few seconds to several minutes in a normal fundus. A short-term decrease in vision can be explained by the occurrence of spasm in the vessels supplying the retina or optic nerve. There are movement disorders (from minor hemiparesis to complete hemiplegia), which may decrease over time.

Treatment.

Patients with vascular pathology of the optic nerve need emergency care. Their treatment should be comprehensive and agreed with the therapist and neuropathologist. The use of vasodilators, decongestants, anticoagulants is shown. Among the latter, heparin deserves special attention.

Being a direct-acting anticoagulant, it simultaneously gives a vasodilating, anti-inflammatory and hyposensitizing effect. Heparin can be administered parabulbarno, subconjunctival, and also intramuscularly or subcutaneously. Recommended anti-sclerotic agents (diasponin, cetamiphene, iodine preparations, etc.), vitamin therapy (vitamins of group B), ATP, distraction and resolving therapy, oxygen therapy.

Along with this, in vascular processes caused by temporal arteritis, and in vasculitis of the optic nerve, corticosteroids and hyposensitizing agents are indicated.

Forecast

in vascular diseases of the optic nerve is always serious, but not hopeless. In some cases, under the influence of treatment, improvement or stabilization of the disease process may occur. However, it is not always persistent, so repeated treatment is required in the form of regular courses.

The development of ischemic optic neuropathy occurs due to a local change in blood flow, as a result of which local tissues do not receive enough nutrients. The formation of pathology is associated with the course of atherosclerosis, hypertension, diabetes mellitus or cardiac dysfunction.

Neuropathy is characterized by impaired peripheral vision (narrowing of the visual field), the appearance of blind areas (cattle). Treatment of the disease is carried out with the help of conservative (drug) therapy and magnetic therapy methods.

Ischemia of the optic nerve

Ischemic neuropathy (neuroopticopathy) of the optic nerve is considered the most common cause of blindness. This pathology is most common in people over 50 years of age. The disease is more often diagnosed in men.

Pathology does not develop in isolation, but is a complication of diseases of the eyes or other organs.

In this regard, if optic nerve ischemia is suspected, a comprehensive examination of the patient is carried out in order to identify the cause of nerve fiber damage.

How is it classified?

At the site of the pathological process, the anterior and posterior forms of ischemic neuropathy are distinguished. Depending on the degree of damage to the optic nerve, the disease is divided into local (limited) and total.

The development of anterior ischemic neuroopticopathy is associated with impaired blood flow in the intrabulbar region. Such problems are caused by various forms of vascular lesions: thrombosis, embolism, spasm. With posterior ischemic neuropathy of the optic nerve, the pathological process is localized in the retrobulbar region (behind the eyeball). The anterior form develops less frequently.

Causes of ischemic neuropathy

The disease develops against the background of systemic pathologies that affect the vascular bed and cause a disorder of blood microcirculation. In this case, the influence of local disturbances cannot be excluded. The latter include spasms (functional disorders) and organic lesions (thrombosis, sclerotization) of local arteries.

Systemic vasculopathy leads to the development of eye ischemia:

  • hypertonic disease;
  • atherosclerosis;
  • diabetes;
  • thrombosis of the main vessels;
  • systemic lupus erythematosus;
  • arterial hypertension.

The appearance of anterior ischemic neuropathy may be associated with the course of vasculitis:

  • syphilis;
  • damage to the carotid arteries of an occlusive nature;
  • giant cell temporal arteritis;
  • allergic vasculitis;
  • other vasculitis.

Possible causes of this pathology of the optic nerve include:

  • heavy blood loss (including during surgery);
  • anemia
  • discopathy of the cervical spine;
  • glaucoma;
  • surgery for cataract removal.

Damage to the cervical spine and carotid arteries mainly causes the development of posterior ischemic optic neuropathy.

Symptoms

The course of ischemic optic neuropathy is more often unilateral. Only in 30% of cases the disease affects both eyes. Moreover, bilateral eye damage is usually observed in those patients who have not undergone timely treatment of anterior ischemic neuropathy (neuropticopathy).

Most patients are diagnosed simultaneously with both forms of the disease.

Along with this pathology, lesions of the retinal vessels are noted.

Symptoms of ischemia of the optic nerve appear suddenly. Initially noted:

  • a sharp decrease in visual acuity;
  • violation of light perception;
  • complete blindness (in case of total defeat).

These phenomena are temporary, and after a few minutes or hours, visual functions are restored on their own. In some cases, before the onset of these signs, symptoms-harbingers are noted:

  • blurred vision;
  • pain behind the eyeball;
  • intense headaches.

Regardless of the form of pathology, with ischemic neuropathy of the optic nerve, the quality of peripheral vision decreases, manifested as:

  • livestock (blind spots in the field of view);
  • concentric narrowing of vision;
  • loss of the lower, temporal or nasal region of the field of view (the patient does not see objects from the side of the temples, jaw or near the nose).

Clinical phenomena characteristic of the acute stage of the development of the disease disturb about 4-5 weeks. At the end of this period, the swelling of the optic nerve subsides, and internal hemorrhages in the region of the eyeball resolve without intervention.

At this stage, atrophy of the optic nerve of varying severity develops and visual acuity (especially peripheral) is not restored. Often, at the end of the acute period, the disease progresses.

Diagnostics

Due to the fact that ischemic neuropathy develops against the background of various diseases, the diagnosis of this disease is carried out by:

  • ophthalmologist;
  • endocrinologist;
  • neurologist;
  • rheumatologist;
  • cardiologist;
  • hematologist.

If ischemic optic neuropathy is suspected, the following studies are prescribed:

  • functional tests;
  • ophthalmological examination of the intraocular fundus;
  • electrophysiological studies;
  • radiography.

Studies in anterior ischemic neuropathy show a decrease in visual acuity of varying degrees.

Ophthalmoscopy reveals swelling of the optic nerve and localization of its damage.

The veins are narrowed in the central part, and expanded along the edges. In rare cases, the examination reveals internal hemorrhages.

The following studies are considered important from the point of view of making a diagnosis and determining the causes of the lesion:

  • color Doppler mapping of the eye;
  • ultrasound duplex scanning of the carotid arteries;
  • daily monitoring of blood pressure indicators;
  • biopsy of the temporal arteries;
  • MRI of the brain.

In addition to these methods, angiography of the retinal vessels is often used, through which the nature of their damage is determined. Conducting ultrasound helps to identify the features of changes in blood flow in the arteries.

With the help of electrophysiological techniques, the nature of the decrease in the functionality of the optic nerve is diagnosed. In order to identify the causes of the disease, these studies are supplemented by a coagulogram that helps determine the level of cholesterol and lipoproteins.

The above examination methods help to differentiate this pathology with tumors of the central nervous system and.

Treatment

In case of detection of ischemic neuropathy of the optic nerve, treatment should be started immediately.

Long-term circulatory disorders cause the death of nerve fibers, which is irreversible and cannot be eliminated even through surgical intervention.

Immediately after the detection of pathology are assigned:

  • intravenous infusion of a solution of "Euphyllin";
  • taking nitroglycerin (placed under the tongue);
  • inhalation over vapors of ammonia.

The main goal of treatment is to ensure a stable remission of the disease and prevent the spread of neuropathy to the second eye. In this regard, the therapy of pathology is carried out in a hospital setting.

For example, signs of ischemic optic neuropathy are a sharp deterioration in vision, usually in one eye. In some cases, ischemic blindness may occur. Often this is preceded by such specific symptoms as, for example, blurred vision, the appearance of various spots, sometimes colored.

If an incomplete loss of vision has developed, then focal loss of visual fields appear, for example, arcuate and sectoral scotomas, that is, areas of visual fields that do not see anything. A concentric narrowing of the visual fields may appear.

This process is very dangerous with the so-called sympathetic spread: in some cases, the pathological process is transferred from one eye to the second (after all, the optic nerves form a single whole in the region of the chiasm, or optic chiasm), and as a result, complete blindness may develop.

With ischemic lesions of the nerve, disc edema also occurs, and the arteries narrow, with a normal diameter of the veins. This is clearly seen when examining the fundus. Then, various hemorrhages occur in the region of the optic nerve head. In the event that intensive treatment is not started (metabolic, vascular drugs, antithrombotic, antiplatelet agents, antioxidants), persistent atrophy of the optic nerve may develop. It usually occurs 1-3 weeks after the onset of neuropathy.

This lesion (ischemic neuropathy of the optic nerve) occurs with severe atherosclerosis, systemic vascular lesions - cranial arteritis. It is also possible to damage the optic nerve with obliterating endarteritis, with Buerger's disease (thromboangiitis obliterans).

If we talk about optic neuritis, then it occurs due to the appearance of inflammation on the myelin sheath, as well as in the nerve trunk itself. Signs of optic neuritis will be exclusively signs that are noticeable during the examination of the fundus:

  • hyperemia of the nerve disk, swelling;
  • blurring and fuzziness of the boundaries of the disk, which indicates inflammation;
  • plethora and a sharp expansion of both arteries and veins (and we remember that with neuropathy there is the opposite, narrowing of the arterial network with an intact venous network. It is clear that plethora is a sign of inflammatory hyperemia);
  • foci of hemorrhage, in the disc area;
  • the appearance of whitish foci on the surface of the disc and retina.

Signs of optic neuritis will also be a variety of visual disturbances, including early loss of acuity, as well as wide and varied changes in visual fields. These disorders occur simultaneously with the appearance of the picture on the fundus.

Tellingly, visual disturbances when using methanol occur 3-6 days after ingestion, when, it would seem, everything has returned to normal. Damage to the optic nerve after taking methanol occurs due to the fact that in the liver it breaks down into toxic products - formic acid and formaldehyde. It is the latter that affects the optic nerve. When using ordinary, ethyl alcohol, the products of metabolism in the liver are acetic acid and acetaldehyde, which, for all their harmfulness, do not affect the cells of the optic nerve and retina.

Optical neuropathy

Optic neuropathy is the name given to damage to the fibers of the optic nerve, which is accompanied by its atrophic degeneration with the development of characteristic clinical symptoms. Previously, this condition was referred to as "optic nerve atrophy", but at present, ophthalmologists recommend not to use it.

Optic neuropathy is not a spontaneously occurring pathology, but one of the manifestations or a consequence of a number of diseases. Pathogenesis

Whatever the cause of damage to the optic nerve, ischemia of the nerve fibers with a weakening of the antioxidant defense mechanism are the key pathogenetic moments. This can be facilitated by various etiological mechanisms:

If the damage becomes irreversible and progressive, the nerve fibers die and are replaced by glial tissue. Moreover, the pathological process tends to spread, so optic neuropathy in most cases tends to increase. The area of ​​appearance of the primary focus and the rate of degeneration (atrophy) of the nerve depend on the etiology (cause).

What causes optic nerve degeneration

There are a lot of diseases that can provoke the development of optic neuropathy. According to the mechanism of damage to the optic nerve, all of them can be divided into several groups:

  • Diseases with a predominantly vascular pathogenetic factor. These include diabetes mellitus, hypertension and secondary arterial hypertension of any origin, arterial hypotension, temporal arteritis, generalized atherosclerosis, periarteritis nodosa, thrombosis of the main vessels of the cervicocerebral region and arteries supplying the nerve.

  • Infiltration of the optic nerve trunk. Most often we are talking about germinating tumors, which are primary and secondary (metastatic). Foci of inflammation, sarcoidosis, and a fungal infection can also lead to nerve infiltration.
  • Demyelinating diseases (multiple sclerosis). The exposure of nerve fibers leads first to a violation of the conduction of impulses along them, and then to irreversible degeneration.
  • Toxic form of optic neuropathy. Damage to the optic nerve may be associated with exposure to a number of industrial toxins, toxic substances, pesticides, alcohol and its surrogates. The greatest danger is methyl alcohol, whose metabolites (especially formaldehyde) are highly toxic and have a tropism for the optic nerve. Optic neuropathy can develop while taking certain drugs: for example, with intolerance to sulfonamides, with a severe overdose of cardiac glycosides and amiodarone, during the treatment of tuberculosis with ethambutol.
  • Optic nerve dystrophy, caused by severe chronic hypovitaminosis, and a long-term deficiency of B vitamins is especially critical. One of the most common causes of optic neuropathy is glaucoma. The pathogenesis of this disease includes the gradual death of the retinal structures due to its chronic compression in the deformed cells of the supporting cribriform plate of the sclera and the inclusion of the vascular component. Clinical manifestations

On average, the optic nerve contains about 1–1.2 million neuronal fibers, each of which is covered with a myelin sheath. This structure provides isolation of the conducted pulses and increases the speed of their transmission. It is the defeat of these fibers in any part of the nerve that most often causes the appearance of symptoms, regardless of the etiology of the process and the location of the primary focus of degeneration.

The main clinical manifestations of optic neuropathy include:

  • Reduced visual acuity, and this violation is not amenable to adequate correction with glasses / lenses. Initially, patients may report blurred vision.
  • Change in color perception.

Features of some forms of optic neuropathy

Despite the uniformity of symptoms, optic neuropathy of various origins has some features.

  • In posterior ischemic neuropathy, symptoms usually increase gradually and asymmetrically. Their appearance is associated with a chronic violation of the blood supply to the intraorbital part of the optic nerve against the background of damage to the carotid arteries and their branches. Therefore, this variant of optic neuropathy is more common in older people suffering from atherosclerosis, arterial hypertension, and diabetes mellitus. Other vascular factors may also be present.   • Disaggregants to improve the rheological properties of blood and reduce the risk of thrombotic complications.
  • Vitamin therapy, it is advisable to use neurotropic vitamins of group B.

Oxygen therapy is also used whenever possible. In the recovery period, laser therapy, magnetic and electrical stimulation of the optic nerves are indicated. And the identified vascular factors (atherosclerosis, arterial hypertension, hypotension, etc.) are subject to correction.

In other forms of optic neuropathy, the etiological factor is also necessarily affected. For example, with endocrine ophthalmopathy, stabilization of the hormonal status is of paramount importance. With post-traumatic compressions, they try to remove foreign bodies and restore the physiological shape of the orbit, to evacuate large hematomas.

Forecast

Unfortunately, the symptoms of optic neuropathy are rarely completely reduced even with early adequate therapy. Good results include partial restoration of vision and the absence of a tendency to progression of symptoms in the long term. In most patients, defects in peripheral vision and a decrease in visual acuity persist, which is associated with the development of irreversible nerve atrophy. And chronic vascular neuropathy is usually prone to slow and steady progression.

After relieving the severity of the condition and stabilizing the symptoms, the prevention of recurrent ischemic attacks and the containment of the process of neurodegeneration are of paramount importance. Most often, long-term maintenance therapy is prescribed, aimed at preventing thrombosis, improving the blood lipid profile. Often, repeated courses with the use of vascular drugs are recommended, and in the case of endocrine ophthalmopathy, the patient is referred to an endocrinologist for adequate correction of existing disorders.

The main sign of optic nerve atrophy is a decrease in visual acuity that cannot be corrected with glasses and lenses. With progressive atrophy, a decrease in visual function develops over a period of several days to several months and may result in complete blindness. In the case of incomplete atrophy of the optic nerve, pathological changes reach a certain point and do not develop further, and therefore vision is partially lost.

With atrophy of the optic nerve, visual dysfunctions can be manifested by a concentric narrowing of the visual fields (disappearance of side vision), the development of "tunnel" vision, a color perception disorder (mainly green-red, less often blue-yellow part of the spectrum), the appearance of dark spots (cattle) in areas fields of view. Typically, an afferent pupillary defect is detected on the affected side - a decrease in the pupil's reaction to light while maintaining a friendly pupillary reaction. Such changes can be observed in one or both eyes.

In children, optic nerve atrophy can be either congenital or develop later. In the first case, the child is already born with impaired vision. You can notice the impaired reaction of the pupils to light; also draws attention to the fact that the child does not see objects brought to him from a certain side, no matter how close they are not located from his eye (s). Most often, a congenital disease is detected during a routine examination by an ophthalmologist, carried out at the age of up to a year.

Optic nerve atrophy, which occurs in children aged 1–2 years, can also go unnoticed without undergoing a routine examination by an ophthalmologist: children of this age do not yet understand what has happened and cannot complain.

In some cases, attention is drawn to the fact that the child begins to rub his eyes, turn to the object in some way.

Symptoms in older children are the same as in adults.

With timely treatment, if it is not a genetic disease in which there is an irreversible replacement of nerve fibers with connective tissue, the prognosis is more favorable than in adults.

Atrophy of the optic nerves with tabes and progressive paralysis has the character of simple atrophy. There is a gradual decrease in visual functions, a progressive narrowing of the field of view, especially in colors. Central scotoma is rare. In cases of atherosclerotic atrophy resulting from ischemia of the optic disc tissue, there is a progressive decrease in visual acuity, concentric narrowing of the visual field, central and paracentral scotomas. For atrophy of the optic nerve due to sclerosis of the internal carotid artery, nasal or binasal hemianopsia is typical. Hypertension can lead to secondary atrophy of the optic nerve due to hypertensive neuroretinopathy. Changes in the visual field are varied, central scotomas are rare.

Atrophy of the optic nerves after profuse bleeding (often gastrointestinal and uterine) usually develops after some time. After ischemic edema of the optic nerve head, secondary pronounced atrophy of the optic nerve occurs with a significant narrowing of the retinal arteries. Atrophy of the optic nerve from compression caused by a pathological process (more often a tumor, abscess, granuloma, cyst, chiasmatic arachnoiditis) in the orbit or cranial cavity usually follows the type of simple atrophy. Changes in the visual field are different and depend on the location of the lesion. At the beginning of the development of atrophy of the optic nerves from compression, there is often a significant discrepancy between the intensity of changes in the fundus and the state of visual functions.

With mildly pronounced blanching of the optic nerve head, a significant decrease in visual acuity and sharp changes in the visual field are noted. Compression of the optic nerve leads to the development of unilateral atrophy; compression of the chiasm or optic tracts always causes a bilateral lesion.

Family hereditary atrophy of the optic nerves (Leber's disease) is observed in men aged 16–22 years in several generations; transmitted through the female line. It begins with retrobulbar neuritis and a sharp decrease in visual acuity, which after a few months pass into primary atrophy of the optic nerve head. With partial atrophy, functional and ophthalmoscopic changes are less pronounced than with complete atrophy. The latter is characterized by a sharp blanching, sometimes a grayish color of the optic disc, amaurosis.
