Autoimmune thyroiditis is a fairly common disease of the thyroid gland. The second name of this disease is Hashimoto's thyroiditis, after the name of the scientist who first described its symptoms.

With AIT, inflammatory lesions appear in the tissues of the organ as a result of the fact that its own immune system, for some reason, begins to destroy thyroid cells. All this leads to a violation of the hormonal balance of the body.

The reasons

It has now been proven that there is no human fault in the occurrence of this disease. AIT can develop as a result of:

  • hereditary factor;
  • severe stress conditions;
  • depending on gender (in women it occurs much more often than in men);
  • poor environmental situation;
  • progressive infectious diseases.

Other reasons can lead to the development of this disease, because each person is an organism that differs in its individuality. So the cause can be a lack or, conversely, an excess of iodine.

Symptoms of autoimmune thyroiditis

As a rule, Hashimoto's thyroiditis develops rather slowly, so it does not show any special signs at the initial stage. Very often, throughout life, the thyroid gland can continue to function in a full-fledged mode. This condition does not pose a particular danger to humans, but requires constant monitoring of the development of the disease.

However, in most cases, with such a disease, the thyroid gland malfunctions, as a result of which it begins to produce either more or less hormones than the body needs.

As a rule, at the initial stage of AIT, there is an increase in thyroid function, it begins to produce more hormones. This condition is called thyrotoxicosis. In the future, it may persist, or it may lead to the onset of hypothyroidism, i.e. on the contrary, to a decrease in the production of hormones. The symptoms of these two conditions are different in humans.

Thyrotoxicosis

In this state of the body, the thyroid gland produces more hormones than the body needs. It is characterized by the following features:

  • Irritability, aggressiveness, mood swings, insomnia. In addition, a person develops increased fatigue, apathy.
  • Increased appetite, while the weight may not change or even decrease.
  • Severe hair loss and increased fragility of nails.
  • Menstrual disorders in women.
  • Bones become more fragile, which is fraught with frequent fractures.
  • Problems with the gastrointestinal tract, frequent diarrhea.
  • Sweating, intolerance to hot climates.
  • Violation of the heart rhythm.

Hypothyroidism

In hypothyroidism, the thyroid gland, on the contrary, produces an insufficient amount of hormones for the body. It is characterized by its own symptoms of AIT. Here they are:

  • impaired attention, memory loss;
  • hair loss;
  • swelling of the face, limbs;
  • slow speech;
  • excess body weight;
  • violation of the menstrual cycle, infertility;
  • constant feeling of cold;
  • disruption of the heart, the development of coronary disease;
  • constipation;
  • pain in the joints.

The symptoms of autoimmune thyroiditis are generally similar for any thyroid disorder. That's why only a doctor can make an accurate diagnosis after the results of all analyzes are received. By the way, women detect a disease in themselves much earlier precisely because of the sharp hair loss, so they go to the doctor much earlier than men. After all, for them, the loss of their luxurious hair can be a real tragedy.

Kinds

There are several groups of autoimmune thyroiditis:

  • Chronic autoimmune thyroiditis or, as it is also called, Hashimoto's thyroiditis. This disease develops gradually. In this case, primary hypothyroidism first occurs - the production of hormones decreases. AIT in chronic form is most often a genetic disease.
  • Postpartum thyroiditis. It appears as a result of the fact that during pregnancy the body's immune system is suppressed, and after childbirth, its excessive activation occurs. And if there is a predisposition, then the occurrence of AIT is quite possible.
  • Painless. This form is very similar to the previous one, however, its exact causes have not been established.
  • If interferon preparations are used during the treatment of blood diseases and hepatitis C, this can provoke the occurrence of cytokine-induced thyroiditis.

In the last three types of the disease, similar signs develop. At the initial stage, thyrotoxicosis appears, which then with a high probability will turn into hypothyroidism.

Phases

In addition, this disease can be divided into phases in which it develops. Here they are:

  • The first phase is characterized by the normal state of the thyroid gland, its functions are not impaired. This period can be quite long.
  • In the next phase, the cells of the organ begin to break down, resulting in a decrease in the production of thyroid hormone.
  • In the third phase of the disease, a large amount of thyroid hormone enters the bloodstream due to the destruction of thyroid tissue. The result is thyrotoxicosis.
  • Subsequently, due to a sharp decrease in the hormone T4 in the blood, hypothyroidism develops. This is the fourth phase. After about a year, thyroid function can recover, but sometimes hypothyroidism remains for a long time.

Sometimes in humans, the disease can be constantly in one phase, for example, only hypothyroidism or only thyrotoxicosis.

Forms

In addition to the previous classifications, Hashimoto's thyroiditis has several different forms:

  • Hidden (latent). At the same time, the thyroid gland is in a normal state, its dimensions are not changed, its functions are not impaired. A person may have only minimal signs of thyrotoxicosis or hypothyroidism.
  • Hypertrophic. In this form of the disease, the iron increases in size. There is a diffuse form - the organ is enlarged evenly in volume, nodular - nodular formations are detected during the examination, or it can be mixed - a diffuse-nodular form of the disease. At an early stage, all signs of thyrotoxicosis are characteristic, then this condition can gradually turn into hypothyroidism.
  • atrophic form. It is most common in the elderly, while the work of the thyroid gland is sufficiently reduced, so signs of hypothyroidism predominate.

In children

The most obvious sign of autoimmune thyroiditis in children is the appearance of a goiter. However, this does not mean that other symptoms are absent. You can also notice a deterioration in attentiveness, constant fatigue, hair loss, weight gain. If such a disease is not treated, then the consequences for the child will only be negative. The fact is that in such children it is very often possible to observe developmental problems, in addition, the nodal forms of the disease greatly affect the child's condition. Due to a malfunction of the thyroid gland, the body will not be able to fully cope with its functions. In addition, Hashimoto's thyroiditis, if left untreated, can lead to the development of Graves' disease. And this disease is much more dangerous and serious. Therefore, leaving AIT in children without treatment is not worth it.

But today the story is about such a mysterious autoimmune thyroiditis. Currently, there is an increase in autoimmune diseases in general. There are many theories about the causes of AIT. Here are the main ones:

1)viral, this is a provocation of the inflammatory process by Coxsackie and herpes viruses (type 6, etc.);

2)genetic theory prerequisites for immune diseases (as well as bronchial asthma, rheumatoid arthritis, etc.);

3) increased iodine intake in the form of medicines, it provokes and enhances the activity of the autoimmune process in the thyroid gland. Dyspolymicr (macro) elementosis is a violation of the homeostatic balance of the macro-microelement supply of the body, which also provokes and initiates immune inflammation, oxidative stress and other pathological processes that damage the thyroid tissue.

This theory is also confirmed by the fact that the clinical manifestations (symptoms) of autoimmune thyroiditis are not typical for true inflammation that occurs when an infection enters the gland.

The ecological situation in the city or region where a person lives, as it turned out, directly affects the development of AIT, due to an excess of toxic elements: lead, cadmium, aluminum, mercury, etc. in the air, water, soil.

That is why AIT is now called an environmentally dependent disease.

Cadmium, for example, is a direct antagonist of the beneficial selenium and zinc.

Classification of autoimmune thyroiditis

All structural variants of thyroid diseases can be accompanied by autoimmune thyroiditis. This contributes to a significant expansion of the classification series. At the same time, etiological (causal) and pathogenetic (according to the mechanism of action) phenomena are classified as autoimmune processes in the thyroid gland.

The most practically applicable is the division of autoimmune thyroiditis into two types: autoimmune hyperthyroidism and, in fact, autoimmune thyroiditis. If in autoimmune hyperthyroidism the diagnostic search is focused on the detection of AT-rTTH in the blood, then in euthyroid and hypothyroid autoimmune thyroiditis, it is on the determination of AT-TPO and AT-TG.

In addition, autoimmune thyroiditis is classified in a complex manner, in accordance with morphological, etiological, functional, age and other features. Therefore, they distinguish:

  • thyroiditis and / or goiter Hashimoto (Hashimoto);
  • atrophic chronic autoimmune thyroiditis;
  • painless;
  • postpartum;
  • juvenile;
  • senile;
  • cytokine-induced;
  • focal, etc.

  • Researchers define autoimmune thyroiditis in the opposite way. Some experts characterize it as a disease, trying to give this condition the category of disease. Others speak of autoimmune thyroid antibody carriage as a form that is transitional to other diseases of the thyroid gland. In our Clinic, theoretical analysis and practical data allow us to evaluate the immune processes of the thyroid gland as compensatory and adaptive. These autoimmune phenomena are necessarily expected in any degree of exhaustion and overexertion.

    In accordance with the ideas of the first group of specialists, the phases of autoimmune thyroiditis are distinguished: euthyroid, subclinical, hypothyroid, hyperthyroid (thyrotoxic). But the lack of a full-fledged scientific justification for such a multi-phase thyroiditis, together with the empirical linking of immune changes to the supply of hormones to the body, contributes to practical errors and therefore reduces the value of such a classification.

    In the essential Clinical classification proposed by us (Clinic of Dr. A.V. Ushakov, 2010), the autoimmune process is defined as a compensatory phenomenon with varying degrees of activity. In accordance with the titer of antibodies in the blood, a small, moderate and significant degree of the autoimmune process is released. For example, usually, an increase in AT-TPO up to 300-500 U / l is considered to be a small degree, from 500 to 1000 U / l - as a moderate degree, and more than 1000 U / l - as a significant degree. This assessment takes into account the reference data of the laboratory.

    Each degree of activity closely correlates with the magnitude of morphological changes in the gland. Such a classification division allows us to assess the intensity of immune events and determine the prognosis of thyroid disease.

    Today, thyroid diseases are the main reason for turning to endocrinological specialists.

    The current epidemiological situation is based on the increased sensitivity of this internal organ to such adverse factors as environmental pollution, growing background radiation and chronic stress.

    As a result of their negative impact, the normal functioning of the thyroid gland is disturbed, structural changes in the thyroid tissue occur and hormonal dysfunctions develop. One of the most commonly diagnosed disorders of the endocrine system is autoimmune thyroiditis (AIT).

    And, of course, every sane person, upon discovering this pathology, has a reasonable question: “AIT of the thyroid gland - what is it, is there a danger to health, and what effective methods of treating the disease does modern medicine have?”

    What is AIT of the thyroid gland?

    Autoimmune thyroiditis is an organ-specific inflammatory disease of the thyroid gland that develops against the background of an inadequate immune response.

    In this disease, healthy thyroid tissue, perceived as foreign, is destroyed (by no means always!) by its own cells of the immune system.

    Naturally, the destruction of the thyroid gland further leads to the development of primary hypothyroidism.

    Despite the fact that autoimmune thyroiditis is always accompanied by the presence of circulating antibodies to thyroid tissue, it is absolutely wrong to equate these two concepts. Thus, among the female part of the population, the prevalence of carriage of antibodies to thyroid cells reaches 27%, but the development of the disease is observed only in 3–6% of cases. Only in older women (after 60 years) the frequency of hypothyroidism is 15-20%.

    Unfortunately, in medical practice there are still situations in which the presence of antibodies to thyroid tissues, in a minimally elevated titer with an absolutely normal level of thyroid-stimulating hormone (TSH), becomes a formal reason for diagnosing AIT and prescribing a “prophylactic” course of Levothyroxine. The consequence of such an erroneous approach is thyrotoxicosis.

    Symptoms of autoimmune thyroiditis

    Most often, the clinical picture in the early stages of AIT of the thyroid gland is latent.

    So, during palpation of the thyroid gland, it is not possible to detect signs of its increase, there are no symptoms of endocrine abnormalities, and the content of hormones in the blood plasma is within the physiological norm.

    In some cases, the onset of the disease occurs against the background of transient thyrotoxicosis, which is never associated with a hyperfunctional state of the thyroid gland. The pathogenesis is based on either the destruction of follicles and the release of hormones into the blood, or the temporary production of thyroid-stimulating antibodies.

    The thyrotoxic phase lasts 3–6 months. It has a mild course and is usually detected quite by accident when a low TSH concentration is detected. Among its symptoms are:

    • subfebrile condition;
    • small trembling;
    • emotional lability;
    • increased sweating;
    • insomnia;
    • weight loss
    • tachycardia;
    • arterial hypertension.

    Often, patients with an enlarged thyroid gland present the following complaints:

    • increased weakness;
    • fast fatiguability;
    • arthralgia;
    • sensation of pressure and lump in the throat.

    In the final stage of autoimmune thyroiditis, primary hypothyroidism develops, which determines the main clinical manifestations of the disease:

    • decrease in working capacity;
    • swelling of the face;
    • slowing down of speech and thinking;
    • memory impairment;
    • obesity;
    • chills;
    • dysmenorrhea;
    • female infertility;
    • decrease in heart rate;
    • yellowness of the skin;
    • dyspnea;
    • hypothermia;
    • coarsening of facial features;
    • hearing loss;
    • hoarseness of voice;
    • heart failure, etc.

    If the node is large, breathing and swallowing disorders may be disturbed due to compression of the esophagus and trachea.

    Diagnostics

    The "major" diagnostic criteria, the combination of which makes it possible to confirm AIT, include:

    • an increase in the volume of the gland in women and men over 18 and over 25 ml, respectively;
    • detection of antibodies to thyroid tissue in a diagnostically significant concentration and / or characteristic signs of an autoimmune process according to the results of ultrasound (ultrasound);
    • clinical picture of subclinical or overt hypothyroidism.

    It should be noted that the absence of at least one of the above "major" diagnostic features significantly reduces the likelihood of AIT.

    Among the laboratory diagnostic methods, the most informative are:

    • determination of antibody titer to thyroglobulin, thyroperoxidase and thyroid hormones;
    • a blood test for the content of thyroid-stimulating hormone, triiodothyronine (T3) and tetraiodothyronine (T4).

    An important role in endocrinological practice is given to ultrasound of the thyroid gland, which allows determining the position, shape and size of the organ, its echogenicity and echostructure, as well as the presence of tumors.

    If a malignant degeneration of the nodule is suspected, a puncture biopsy is performed, followed by a morphological analysis of the material obtained. By the way, an absolute indication for such a study is the detection of a single node larger than 10 mm (usually accessible for palpation).

    The question of a person's life expectancy when making an oncological diagnosis is very acute. In the case of thyroid cancer, the prognosis is most often favorable. In this topic, we will consider the prognosis of the disease and data on five-year survival after treatment.

    AIT of the thyroid gland - treatment

    Therapeutic tactics in the management of patients with various pathologies of the thyroid gland is based on a differentiated approach. And autoimmune thyroiditis is no exception here.

    Most leading experts in the field of thyroidology consider it rational to start therapy for AIT only if there are signs of a hypofunctional state of the thyroid gland.

    Until this moment, endocrinologists advise to adhere to expectant tactics with dynamic monitoring of patients and annual ultrasound.

    This recommendation is due to the fact that today there are no drugs that could stop or at least slow down the destruction of thyroid tissue with the subsequent development of hypothyroidism.

    As mentioned earlier, sometimes the debut of the disease is hidden under the mask of thyrotoxicosis, which can lead to the erroneous prescription of thyreostatics: Thiamazole, Mercazolil or Propylthiouracil, because an increase in the concentration of T3 and T4 in AIT is not associated with hyperthyroidism. To avoid serious endocrine disorders due to improper treatment, it is important to conduct a thorough medical examination of each patient.

    The outcome of long-term autoimmune thyroiditis is hypothyroidism, the effective correction of which is possible only with the help of hormone replacement therapy.

    If with overt hypothyroidism (T4 is lowered) Levothyroxine is immediately prescribed, then with its subclinical form (T4 is normal), it is recommended to re-examine the hormonal status after 3-6 months.

    Indications for treatment with thyroid hormone preparations in subclinical hypothyroidism are:

    • a single increase in the level of TSH in the blood plasma above the mark of 10 mU / l;
    • at least two-fold detection of TSH concentration in the range of values ​​from 5 to 10 mU/l;
    • treatment of older patients (after 55–60 years);
    • the presence of cardiovascular pathologies.

    In the last two cases, hormone replacement therapy is used only if there is no history of evidence of a deterioration in the state of health in concomitant chronic diseases against the background of its use, as well as with good tolerability of drugs.

    If antithyroid antibodies and / or echographic signs (reduced echogenicity of the thyroid gland according to ultrasound) of autoimmune thyroiditis in women planning pregnancy are detected, a hormonal blood test (tests for TSH and free T4) is necessary not only before the onset of conception, but throughout the entire period bearing a child. If hypothyroidism is diagnosed directly during pregnancy, then Levothyroxine in a full replacement dose should be administered immediately.

    Surgical treatment for AIT is used in exceptional cases. For example, the question of surgical intervention is raised with a significant volume of the thyroid gland, when life-threatening conditions arise due to mechanical compression of the trachea and esophagus.

    • detection of antibodies to thyreglobulin (AT TG) in the blood

    What does "autoimmune" mean? If the word "immune" is more or less clear, then "auto" can be translated from Latin as "own". As a result, we get some process directly related to the immune system of our own body. With the concept of "thyroiditis" everything is also quite simple: "thyroid-" is associated with the Latin name of the thyroid gland (looking thyroid), and "-it" means inflammation.

    In case of damage to the body by autoimmune thyroiditis, antibodies - substances designed to protect the body from harmful agents, cease to "recognize" their own organ and begin to "attack" thyroid cells.
