Massage of the abdomen is one of the most difficult and difficult to perform. Most often, abdominal massage is prescribed for sluggish digestion, accompanied by constipation.

When massaging the abdomen, several goals are pursued:

1) its contents are driven through the intestines,
2) intestinal peristalsis is stimulated,
3) nerve centers are called to more energetic activity,
4) the secretory activity of the intestinal glands is enhanced, such as the liver, pancreas and small glands embedded in the walls of the stomach and intestines.

Methods of abdominal massage are very diverse. They consist of various types of strokes, kneading, shaking and tapping.

Stroking is carried out: on the entire abdomen, along the colon, in the stomach and liver, on the small intestines and in the lower abdomen in the iliac regions.

Kneading raises and regulates intestinal peristalsis, increases the tone of the muscular walls of the abdomen and promotes the absorption of fat in the skin of the abdomen, therefore, kneading techniques are divided into different types of kneading along the colon, kneading on the small intestines, on the abdominal muscles and kneading the skin of the abdominal integuments.

The concussion is carried out locally on the nerve centers that control the work of the entire intestinal apparatus, on the area of ​​​​the stomach, liver, kidneys and bladder and general on the abdominal walls of the entire abdomen.

Tapping is used to cause more vigorous contractions of the smooth muscles that form the walls of the intestines, and to increase blood circulation in them.

For convenience of description, we will divide the study of abdominal massage into areas. The most common indication for the appointment of abdominal massage is the so-called. habitual constipation, having as its cause insufficiently vigorous peristalsis in the large intestine. To stimulate their work, massage is carried out along the colon and general kneading of the abdomen.
General rules for performing abdominal massage

Massage of the abdomen, as mentioned above, belongs to the most difficult in terms of its techniques, requires an attentive and caring attitude and adherence to some very important rules, namely:

1. The movements of kneading and stroking are carried out slowly and smoothly, according to the slow contractions of the smooth muscles that make up the walls of the intestinal canal. Fast movements irritate the muscles and bring them into a state of tension, while the rest of the slow, coinciding with intestinal peristalsis, contribute to more vigorous movements.

2. Movement should penetrate deep into the abdominal cavity. To do this, with all kneading, it is necessary to ensure that the abdominal walls under the hands of the massage therapist follow the movement. If this rule is not observed, abdominal massage is carried out extremely superficially, and may cause skin irritation.

3. Massage of the abdomen should be done with an empty bladder and intestines. A patient suffering from constipation should put an enema before the massage. Deep kneading can act on the walls of the intestines. If the intestines are free, the movement penetrating deep will have the same effect on both the anterior and posterior walls of the intestines. When the intestines are full, only the anterior wall of the intestine is kneaded, at the same time, the mucous membrane rubs against the solid masses in its cavity, which can adversely affect its blood circulation and nutrition. In particular, this rule is important to keep in mind if you have to start massaging the abdomen after a person has not had a stool for two or three days.

4. You should not massage on an empty stomach at all, just like immediately after a very hearty meal. The best time for a belly massage is in the morning, after morning tea and a light morning breakfast. If the massage is carried out after lunch, then the interval should be equal to 3-4 hours.

Not all the techniques described are applied during one massage session. Stroking and kneading the abdomen serve to mechanically empty the intestines, shaking and tapping affect blood circulation and tissue nutrition, and nervous activity.
Abdominal wall massage

The patient lies on his back, his head is raised, a roller is placed under his knees.
The following massage techniques are used:
a) stroking - gentle circular, then planar, starting from the navel, then over the entire surface of the abdomen, always clockwise;
b) rubbing - sawing, hatching, crossing (grinding);
c) kneading - longitudinal, transverse, felting, rolling - depending on the indications;
d) vibration types.

When performing abdominal massage, the patient assumes a semi-recumbent position with a slightly raised head lying on a pillow and bent knees. The massage therapist must place the couch or bed so as to have free access to the patient from both sides.
To perform strokes in the direction of the colon, the massage therapist stands or sits on the right side of the patient; puts the left hand in the right iliac region of the patient, the right hand in the left hypochondrium. Starting stroking from the right iliac region, raises the palm of the hand with outstretched fingers up; having reached the right hypochondrium, he turns his hand and leads it to the left hypochondrium across the abdomen; at the same time, the right hand is carried from under the left hypochondrium down to the left iliac region, where it ends its movement. The left hand descends along the just completed path of the right hand, that is, it also goes from top to bottom, from the hypochondrium to the left iliac region to its innermost corner. Here the movement for both hands ends, a new one begins again with the left hand from the right iliac region, the right hand from the left hypochondrium. Such stroking is carried out several times.

In many patients, at the beginning of the massage, the abdominal walls tighten and make it impossible for the fingers to penetrate deeply into the abdominal cavity. In order to loosen the abdominal walls, Prof. Goffa recommends doing what he calls circular stroking with both hands. Starting the movement from the pubic fusion and leading it from right to left in a clockwise direction along the entire abdomen, the hands, following one after the other, describe concentric circles that enter into each other. After stroking along the colon, various types of deep kneading are carried out, designed to loosen the fecal masses in the large intestine and drive them towards the rectum.

Kneading along the colon begins on the right side of the abdomen. The masseur stands on the right hand of the patient, both hands work: the right hand crushes, kneads the contents of the large intestine, the left hand drives away. The movement is carried out slowly and deeply, four fingers on each hand work, the thumbs serve as a support, and at the same time help to ensure that the loops of the small intestines are held between them and the rest of the massaging fingers, which is why the walls of the large intestines are mainly kneaded and the massaging fingers penetrate very deep into the abdominal cavity.

Starting kneading in the right iliac region, the massage therapist slowly moves forward and up to the right hypochondrium, from there turns his hands and leads them to the left hypochondrium, the fingers of the right hand make a semicircle from right to left, following the course of the peristaltic movements of the large intestine, the fingers of the left hand follow the movement of the right hand . Having completed the semicircle, the fingers stop and begin a new movement in the same direction. Having reached the left hypochondrium, the masseur slightly bends his fingers in the metacarpophalangeal joints and, penetrating deep into the left iliac region, makes a movement, but in the direction from top to bottom.

When kneading in the right iliac region, to relax the abdominal muscles and in order to make the movement deeper, the patient is placed on the right side. When kneading along the transverse colon, the patient lies on his back, and for kneading the colon and S-shaped curvature, lies on his left side. After these kneading, the masseur conducts several deep strokes of the left iliac region, using the ulnar edge of the right hand for this. Stroking is simple and tremulous.

More energetic than the previous movement is circular kneading. The masseur lays his hands on top of each other so that the right hand lies below, the left above it, and begins to knead them in the right iliac region. The first circles described by the fingers are carried out easily, as if by feeling, and only as the patient gets used to the movement, the pressure increases more and more, the patient must breathe deeply to weaken the abdominal muscles. The movement goes in the form of circles, and the massaging fingers drive the contents of the large intestine in front of them from the right iliac region, up to the S-shaped curvature. All work is carried out by the masseur with the shoulder joint, the elbow, wrist and interphalangeal joints are kept almost in a straightened position.

These kneadings are repeated several times and are accompanied by stroking. To first empty the lower intestine, the same kneading can be started from the left iliac region. Putting the hands superimposed one on the other in the left iliac region, the massage therapist, making circular kneading, rises to the left hypochondrium, from here he leads his hands to the right hypochondrium, in which the contents of the caecum are kneaded and without changing the position of the hands, starts stroking upwards from the right iliac region to the right hypochondrium, here he turns his palms across the abdomen, with the fingers turned to the left, and strokes along the transverse colon up to the left hypochondrium. In the left hypochondrium, the hands turn again and stroke from the left hypochondrium to the left iliac region. With this deep stroking, the fecal masses, loosened by previous kneading, are driven into the rectum.

The kneading, which begins in the left iliac region, at first glance seems to be irrational, because it is carried out along the path of the reverse peristalsis of the intestines. In fact, it is quite expedient in that it kneads the underlying masses first and clears the way for the overlying ones. It does not interfere with the peristalsis of the intestines, since those circular movements with which the hands move are carried out with tension along the peristalsis and freely and easily in the opposite direction.

In people with thick abdominal integuments, kneading is done along the colon with the right hand folded into a fist. The right hand, gathered into a fist, works with the middle phalanges of four adjacent fingers, making circular kneading in the course of peristaltic movements, while the left hand, with four outstretched fingers, following the right, makes the same stroking movements as in the described first type of kneading.

Kneading the entire abdomen is carried out with the palm of one or both hands, it is more convenient to knead with both hands, especially if the stomach is large, with thick covers. To do this, the massage therapist sits on the right hand of the patient, puts his palm flat on the stomach, fingers are extended and lie across the abdomen, the thumb is set aside and lies in the right hypochondrium, the base of the hand is above the pupart ligament, the base of the thumb is in the right iliac region. The hand should work softly and strongly, the joints of the working hand should not be tense, the position of the hand and the whole arm in relation to the stomach should be parallel. The movement of the palm is circular, the initial path of movement from the base of the hand. The hand is bent at the wrist joint to the back and moves under gradually increasing pressure to the right into the iliac cavity and from there upward. The thumb moves simultaneously and reaches the right hypochondrium, from there the hand moves to the left, continuing in a circular motion and applying pressure with the thumb and elevation of the thumb, while the hand is in a pronation position, the thumb is slightly adducted to all the others.

When moving to the left hypochondrium and the left iliac region, the fingers bend in the metacarpophalangeal joints, and the hand bends to the ulnar side, when the hand approaches the ilium, it assumes a supination position, having reached the pubic fusion, the hand takes its original position, and the movement begins again. Thus pass through the entire abdomen several times. When kneading with both hands, the left hand is superimposed on the right and helps her in work. Kneading of the abdominal walls can be carried out with both hands, starting from both iliac regions. The masseur becomes facing the patient, puts his right hand on the left side of the abdomen, left on the right, so that all fingers wrap around the stomach on the sides, and the pulp of the palm and thumb knead the abdominal walls, and the left hand in the right hypochondrium makes a circular motion from the bottom up , right from top to bottom, making such short circular movements, the hands rise from both iliac regions to the hypochondria, and the abdominal walls are rubbed between the palms.

In general, the methods of general kneading of the abdomen are very diverse. Also recommended are kneadings called helical, transverse and serpentine. With helical kneading, the masseur puts his hands in the right iliac region at an angle to the course of the fibers of the external oblique muscle and, grabbing the underlying tissues between the working hands, rolls them from right to left, gradually moving forward and to the left. The movement is undulating. The brush then descends with the fingers deep into the abdominal integument, then with the palm, namely, the elevation of the thumb. The same wave-like movement with the palms and fingers is also used for transverse kneading. With transverse kneading, the movement also begins in the right iliac region, the palms with outstretched fingers lie across the abdomen and, making wavy translational movements, reach the left iliac region, where the hands turn and begin to work with the ulnar edge. Serpentine kneading, a movement of the same type, with which the hands are not superimposed on the stomach in parallel, but follow each other, the right hand lies in the right iliac region across the abdomen, the left one follows it first. The movement is also progressive-wave-like, going along the transverse colon and ends with kneading the left iliac region.

To strengthen the abdominal walls, the abdominal muscles are kneaded.
In order to cause a more energetic work of the intestines, after various methods of stroking and kneading along the colon and on the entire abdomen, the entire abdomen and its various departments are shaken, chopped and punctured.

Trembling pressure along the colon is carried out with a hand gathered into a fist. The left hand rests on the right shoulder of the patient, while the right hand, gathered into a fist, begins a concussive pressure in the right iliac region and gradually moving from one place to another, conducts trembling pressure along the entire colon.

If there is a tendency to constipation, but in the absence of abnormal mobility of the liver and kidneys and in general diseases of the abdominal organs, the abdomen is shaken before the end of the massage. To do this, the massage therapist stands facing the patient, takes him with both hands under his back and shakes the patient's stomach with a trembling movement. Having brought his hands out, the massage therapist again puts them under his back to repeat this movement from 8-10 times.

You can combine this shaking with grinding or kneading the abdominal walls. Putting his hands under the back of the patient, the masseur wraps them around the side walls of the abdomen and, working alternately with one or the other hand, moves slowly from back to front, kneading the side walls of the abdomen, then carries out the same trembling movement and shaking the abdomen, gradually freeing his hands.

Chopping along the abdominal walls can be performed in all directions. The fingers are kept bent while chopping, the blow should be soft and very gentle.

Another type of chopping of the abdomen consists in tapping with the back surface of the fingers bent in the first phalanges. The wrist joint is slightly bent, the fingers in the metacarpophalangeal joints and in the last interphalangeal joints are slightly bent, in the first interphalangeal joints they are bent at a right angle, which is why the second phalanges are released, and they tap the abdominal walls in various directions. The blows are soft, quickly follow each other.

Tapping of the abdomen should be carried out with great care, very lightly and for a short time.

There are experiments by professor of physiology Goltz, in which he stopped the heartbeat of a frog with blows to the stomach, a similar phenomenon can occur in humans with strong tapping.

As a calming movement, after all massage techniques, palmar transverse stroking is carried out for patients. For this stroking, the patient keeps his legs extended, the masseur stands on the left side of the patient, puts both hands flat on his stomach, one next to the other, and leads them from the right hypochondrium to the left. Without removing his hands, he freely, without applying pressure, again shifts them to the right hypochondrium, from there with pressure to the left, repeats the movements 8-10 times and finishes the massage of the abdomen with them.
Small intestine massage

The procedure is performed by stroking with fingers, intermittent vibration with the ends of bent fingers, pressing with the palm of your hand (you can use the fingertips) from right to left clockwise over the entire surface of the abdomen. Massage occurs reflexively due to the joint innervation of this area and the small intestine.

Massage in the area of ​​the small intestines is carried out more easily than in the colon. The umbilical region is subjected to stroking, kneading and shaking.

Stroking is carried out with both hands, working alternately and conducting circular movements in the clockwise direction, from right to left. Kneading is done palmar. One or both hands are superimposed on the navel area and starting from the right side, alternate pressure is applied with the palm of the hand, then with slightly bent fingers. When the fingers are lowered deep into the abdominal walls, the palm rises, the next moment pressure is applied with the palm, the fingers move away from the abdominal integuments. Slowly rising and falling, the masseur's hands go around the entire umbilical region. Kneading is carried out with one hand or both together, while the left hand rests on the right, as in circular kneading.

Concussions in the area of ​​the small intestines are divided into general and local. With general concussions, the entire palmar surface of the hand lies on the umbilical region, trembling from the elbow joint is transmitted to the hand, and the entire abdominal surface of the abdomen lying above the hand comes into concussion, which is transmitted to the deeply embedded walls of the small intestines.

Another type of general concussion is carried out as follows: the masseur turns his hands with his palms to the face of the patient, folds his hands one with the other with the elbow edges, stretches his fingers and the pulp of four fingers, from the second to the fifth on each hand, deepens into the abdominal cavity in the hypogastric region. At this time, the masseur slightly bends his fingers, as if lifting the small intestines on them, and conducts a concussion.

In addition to these techniques, when massaging the abdomen in diseases of the small intestines, all the above methods of general kneading of the abdominal walls are used.
Stomach massage

The patient lies first on his back, and later on his right side. Massage is carried out without selective impact on certain muscles. The muscles of the entire abdomen should be relaxed. Find the left midclavicular line - along it the border of the stomach reaches the 5th rib. The lower border in the region of the anterior abdominal wall is located just above the navel: in women by 1-2 cm, and in men by 3-4. Methods are used:
a) intermittent vibration (performed with fingers spaced rake-like) - in the epigastric region on the left and outside, then inside;

c) reflex massage techniques.

Stroking on the stomach is carried out from top to bottom, along the epigastric region itself, or as it is otherwise called the epigastric region and from left to right, leading stroking from the left hypochondrium to the right, along the bottom of the stomach, to the place of its exit into the duodenum.

When stroking from top to bottom, both hands lie in the epigastric region at the lower edge of the sternum, and from here they stroke through the entire epigastric region. Having brought the hands to the navel, they are taken away and a new stroking begins again at the edge of the sternum.

When stroking the fundus of the stomach, the hands lie with the ulnar edge in the left hypochondrium and, following one after the other, lead stroking to the right to the exit of the stomach into the duodenum.

Stroking should be carried out along the left hypochondrium and epigastric region. Beginners often lead strokes much lower. Such stroking captures the transverse colon and, since it is carried out from left to right, it therefore goes in the opposite direction to peristalsis.

Rubbing of the epigastric region is carried out from top to bottom and from left to right. In the epigastric region, rubbing is carried out with two thumbs. From the left hypochondrium, rubbing is carried out with one hand, with four fingers folded together from the second to the fifth. Rubbing is performed deep in the left hypochondrium.

The right hand works, the left free hand, supports the stomach of the patient, or, which is considered useful in massage from the expansion of the stomach, presses with four fingers in the right hypochondrium at the exit of the stomach. Having squeezed and strengthened the stomach at the place of its exit, we get the possibility of a more direct and energetic action on its walls, which are rubbed with the working right hand. Rubbing is accompanied by stroking and various types of shaking.

1. Concussion in both hypochondria with stroking. Both hands rest on both sides of the chest. The patient takes a deep breath, during which the masseur leads his hands down to the hypochondrium, the massage therapist asks the patient to exhale and at this moment brings both of his hands with the elbow edges to both hypochondria and shakes them, penetrating deep into the abdominal cavity.

When you inhale, the diaphragm descends, with it the stomach and liver descend a little, the next moment the diaphragm rises and the deep trembling pressure applied helps the stomach and liver to follow it. Such strokes with trembling are recommended for expansion of the stomach and prolapse of the liver. Stroking and shaking are done vigorously and repeated several times.

2. Rapid circular movements and rubbing of the epigastric region. The masseur lays his palm flat on the left hypochondrium and makes quick circular movements from top to bottom and to the right, according to the location of the fundus of the stomach and its peristaltic movements.

3. Concussion of the fundus of the stomach. The left hand is superimposed on the right, and both hands lie in the left hypochondrium, from which they are led to the right hypochondrium with the help of short shaking movements that quickly follow each other. Having finished the movement at the right hypochondrium at the exit of the stomach, the fingers are again shifted to the left hypochondrium. Such shaking movements are repeated several times.

4. Trembling in the left hypochondrium. The masseur stands on the right, brings his right hand deep into the left hypochondrium, puts the left hand on the patient's right shoulder in order to have support. The hand lying in the left hypochondrium conducts trembling, which is transmitted deep into the epigastric region.

5. Concussion in the epigastric region on the solar plexus. The masseur stands in front of the patient, puts his hands in the epigastric region in the upper third of the line connecting the xiphoid process with the navel, and makes the patient take a deep breath. During exhalation, he inserts outstretched fingers deep into the abdominal cavity and shakes them.
Massage for rectal prolapse

With a prolapse of the rectum, elevation and trembling are performed on the S-shaped curvature. The masseur stands on the right hand of the patient, puts the left hand on the patient's right shoulder, the right hand in the left iliac cavity, making a skin fold so as not to irritate the skin and not press on it, lowers four slightly bent and spaced fingers deep into the pelvic area, grabs them the underlying parts and together with them leads the hand up and inward towards the navel. The movement of the fingers at the same time is insignificant, they are slightly bent and pull the captured loop of the large intestine upwards. With properly done lifting and trembling of the S-shaped curvature, a feeling of being pulled inward is obtained in the anus.

The first to propose such an elevation of the rectum was the Swedish physician Thure Brandt; in this way he managed to cure several patients from prolapse of the rectum. Capturing a part of the S-shaped curvature through the abdominal walls and pulling it out, Tore Brandt also stretched the previously set, protruding, relaxed segment of the rectum. It is known from physiology that a muscle, stretched beyond its normal length, responds to this stimulation with its contraction, which becomes more vigorous and gives a greater shortening. Each new such irritation causes a new shortening of the muscle, which, finally, through such exercises is strengthened and no longer falls out. The treatment of uterine prolapse is based on the same law of muscle exercise to strengthen them. In this case, the massage therapist stretches the muscles of the round uterine ligaments.
Abdominal massage for obesity of the abdominal walls

When massaging the abdomen for obesity, all methods of muscle kneading and general kneading are applicable. In order to cause the resorption of fat in the subcutaneous tissue, kneading the skin of the abdomen is used. The skin is captured in the fold between the two hands, lifted from the muscles and kneaded in all directions, especially a large amount of fat accumulates in the epigastric region, which is kneaded especially carefully, starting to knead in the right hypochondrium and moving to the left.
Another type of kneading the skin is that the skin is lifted with the left hand, and sawing is done on it with the right hand. Starting the movement in the right iliac region, gradually move the hand going first up to the right hypochondrium, then to the side and to the left, across the abdomen to the left hypochondrium, from where they go down. Also performed and cutting the skin of the abdomen. The left hand lifts and holds the skin, while the right hand cuts. Both when sawing and when chopping, the right hand goes to the left, starting to move 4-8 cm from the skin captured and raised by the left hand.

When massaging the abdomen with obesity, they strive to mechanically crush the fat in the subcutaneous tissue and drive its smallest particles through the lymphatic vessels. All methods of abdominal massage for obesity are painful. Many may have bruises on the skin, therefore, between kneading the skin, make circular strokes of the abdomen for the patient, soothing hypersensitivity, do not get carried away with sawing and chopping the skin, but it is better to slowly treat the skin with the general kneading just described, not forgetting to carefully knead not so much the skin of the abdomen in front, as well as the side walls above the iliac bones, where a large amount of fat always accumulates in the subcutaneous tissue during obesity. In Germany, a method of transverse kneading and stretching of the skin of the abdomen is used for obesity.

Both hands lie on both sides of the white line, grab the skin between the thumbs and all other fingers and, without releasing the fingers, lead the hands to the sides, all the while forming a fold of skin between the fingers. Bringing the fingers to the side walls of the abdomen, remove the hands, again grab the skin near the white line and repeat this squeezing and stretching of the skin 6-8 times, starting a new movement each time below the previous one.

Massaging the abdomen for health purposes is reduced to a superficial massage of the anterior abdominal wall.

The masseur's hands during the massage procedure should be warm, as cold touches are unpleasant for the patient and cause involuntary contractions of the muscles of the abdominal wall.

The massage therapist's hands move from the costal arches to the pelvic area. Complete kneading of these muscles is performed in 5-6 passes.
